판피린 3형제는 판피린, 판콜, 판토로 이번에도 제 마음대로 정해보았습니다. 이 중 판토는 인지도가 너무 떨어지는데 판피린, 판콜 단 두형제로만 하기엔 지난 번 게보린 3형제 이야기도 있고 해서 깔맞춤 할라고 억지로 집어 넣어보았습니다. 우선 판피린 성분을 보시죠. 판콜, 판토도 대동소이 합니다. - 아세트아미노펜 300mg - 말레인산클로르페니라민 2.5mg - 무수카페인 30mg - dl-염산메칠에페드린 20mg - 구아이페네신 40mg - 구연산티페피딘 5mg 소염진통제, 카페인, 항히스타민제, 진해거담제로 이루어져있습니다. 아세트아미노펜은 소염진통제, 클로르페니라민은 항히스타민제, 카페인은 카페인이구요. 그 밖에 염산메칠에페드린, 구아이페네신, 구연산티페피딘 등은 진해거담제들로 주로 교감신경을 자극하여 기관지을 확장시켜 가래를 배출하거나 기침 중추를 억제하여 기침을 억제 시키는 역할 등을 합니다. 판피린 3형제는 원래는 종합감기약 ( 감기를 직접 치료하는 약이란게 존재할까요? 그냥 소염진통제와 항히스타민제 등으로 각각의 증상을 경감시키는 약들이죠.) 으로 출시되었으나 할머니 할아버지들이 감기 걸렸을 때도 먹고, 목이 아파도 먹고, 몸살에도 먹고, 허리 아파도 먹고, 무릎 아파도 먹고, 멀미해도 먹고, 컨디션 안좋아도 먹고, 그냥도 먹고 그렇습니다. 전형적인 약물 중독 현상으로 통증을 완화시켜주는 소염진통제와 피로감을 줄여주는 카페인 그리고 중추신경에 작용하는 다양한 진해거담제들 짬뽕으로 먹어서 중독성을 나타내는 것으로 보입니다. 만병통치약이 되어버린 거죠. 이 약물에 중독된 할머니 할아버지들은 판피린 형제들을 박스째 사서 쟁여놓고 먹습니다. 그나마 약국에서 판매하고 있으니 대량으로 사기가 좀 그렇지만 코스트코나 이마트에 진열해두고 팔면 아마 카트 가져와서 듬뿍 담아서 집에 가져가 가족 모두에게 권해서 가족 전체가 약물 중독에 빠질 겁니다. 맛도 어마어마하게 없더만...우리나라에서는 이것보다 중독성이 심한 약들은 다들 의사 처방전이 필요한 전문의약품으로 묶여있습니다. 그나마 다행이라 할 수 있겠네요. 이런 사소한 감기약도 중독 현상이 일어날 만큼 약들이란 정말 무서운 것들입니다. 본인의 의지로 자제할 수가 없어요. 노인들의 약물 중독이 뭐가 문제냐 하면... 소염진통제 ( 애드빌, 이부프로펜, 덱시부프로펜, 맥시부펜, 나프록센, 게보린, 펜잘, 사리돈, 이지엔6, 이브, 모트린- 타이레놀, 울트라셋-마지막 둘은 NSAID계열이 아니고 위장관부작용보다는 간독성이 문제가 됩니다. )의 다른 말은 암살자입니다. 조용하게 사람을 죽인다고 붙인 무시무시한 별명이죠. 소염진통제 자체는 중독성이 별로 없는 편입니다. 그러나 약물 중독 등 여러가지 원인으로 계속 소염진통제를 장기적으로 복용하게 되면 지난 번에 말씀 드린 대로 필연적으로 위장관 부작용을 겪게되고 특히 체력적으로 약한 나이가 많은 노인들은 이런 위장관질환으로 사망에 이르기까지도 합니다. 미국에서 매년 1만명이상이 소염진통제 부작용으로 사망한다는 보고도 있습니다. (20세기 보고이고 이는 과장되었다는 시각도 있습니다. 지난번 바이옥스사태에서 알 수 있듯이 노인사망자들은 그 원인을 판별하기가 쉽지 않습니다. 사망원인이 복합적이기도 하고 이견도 많구요. ) 위장관 출혈을 일으켜 때로는 피를 토하면서 사망하기도 합니다. 일부 노인들은 각종 통증과 염증으로 정상적인 생활이 불가능한데 이런 소염진통제를 먹고 그나마 일상생활을 누리며 살다가 결국 소염진통제라는 암살자에게 살해 당하면서 죽음을 맞이하기도 하는 겁니다. 물론 통증에 시달리면서 고통스럽게 오랜 삶을 사는 것과 소염진통제를 먹고 위장장애을 앓지만 다른 통증에서 벗어나 정상 생활을 하면서 행복하지만 더 짧은 삶을 사는 것 중에 어떤 것이 옳은지는 잘 모르겠습니다. 하지만 거동이 불편할 만큼 통증이 심하지 않는 노인들이나 젊은 사람들이 약물 중독에 빠져 소염진통제를 장기 복용하는 것은 너무나도 위험한 일입니다. 약물 중독으로 이렇게 소염진통제를 장기 복용하는 것은 본인의 수명을 깍아내리는 일이라 생각합니다. 소염진통제로 인한 위장관 부작용은 가역적인 것이라 소염진통제를 끊으면 다시 위장관 부작용은 사라지므로 많이 두려워 할 필요는 없습니다. 아시다시피 소염진통제의 수많은 사람들에게 이로움을 주는 고마운 존재입니다. 다만 무분별하게 장기복용하는 것을 경계하자는 말입니다. 광고처럼 정말 꼭 먹어야 한다면 그 때 소염진통제를 드시고 장기 복용은 피하는게 좋습니다. 소염진통제를 끝까지 먹으면 끝이 안좋아요. 판피린은 알약도 있는데 지금 말씀드린 판피린은 판피린큐로 내복액입니다. 기본적으로 액제로 만들어진 제제는 흡수가 빠릅니다. 우리가 알약을 먹는다고 알약의 고체 성분 자체가 바로 흡수가 되지 않아요. 정제가 부서지고 ( 용출 ) 다시 각각의 개별 분자 형태로 녹아 ( 용해 ) 소장관 벽을 통해 우리 몸으로 흡수된 다음 전신으로 퍼져서 효과가 발휘합니다. (그래서 약을 먹으면서 물을 먹죠) 그런데 기본적으로 액제는 각 약물이 단분자 형태로 녹아져있기 때문에 용출-용해 과정을 거치지 않아 다른 고체형 알약보다 훨씬 빠르게 흡수가 진행되어 효과도 빠르게 나타납니다. 어떤 분이 녹십자의 탁센에 대해서 물어보셨는데 탁센은 겉보기엔 알약으로 되어있지만 실은 액상제제입니다. 탁센은 나프록센250mg으로 일반 나프록센제제(보통 375~500mg) 보다 함량이 낮지만 액상제제로 되어있어서 흡수가 빠르고 제제 특성상 생체이용률 (Bioavailability) 이 높은 편이라서 250mg로 충분하게 효과를 발휘하고 무엇보다도 액상제제라 단분자형태의 나프록센이 캡슐 안에 녹아있는 상태로 있어 약을 먹자마자 흡수가 일어나 빠른 시간 안에 효과가 나타납니다. "액상제제로 되어있어서 흡수가 빠르고 제제 특성상 생체이용률 (Bioavailability) 이 높은 편이라서" 이 내용에 관심이 많은 분들이 계시네요. 약물에 따라 다르지만 액상을 만들 때 용해보조제가 들어가므로 약물의 흡수가 용출에 의한 결정되는 약물들 예를 들면 용해도가 낮고 흡수율이 높은 약물들은 액상제제가 투여할 경우 생체이용률이 급격히 높아집니다. 모든 약물이 용해도가 높고 흡수율이 높은 게 아니므로 용해도가 낮고 흡수율이 낮은 약물일수록 액상제제의 생체이용률이 급격히 올라갑니다. 하지만 논문들에 나오는 Dose가 같은 액상제제와 정제제제와의 비교는 주로 BEtest (생물학적동등성, Bioequivalence Test) 에 대한 내용들 입니다. 이들은 주로 오랜전부터 시판되어 팔리고 있는 약물들이나 임상자료가 있는 약물에 대해 새로운 제형과 비교적 간단한 BEtest를 통해 허가를 내주는 제도로 제약회사 입장에서 쉽게 허가 내는 방법입니다. 주로 AUC, Cmax, Tmax를 기존제제와 비교하며 동등성이 인정되면 판매허가가 이루어집니다. 이 경우 일반적으로 BA값으로 판단하지는 않습니다. Dose가 다르거나 제형의 획기적인 변화가 생겨 BA값이 변한 경우 임상자료와 독성자료 및 각종 PK 자료를 요구합니다. 언급하신 논문들과 달리 BA는 같은 용량에서 비교하는 것이 아니라 IV 투여대비 PO의 AUC 를 비교하는 것으로 Dose dependency 가 있는 구간에서 행하는 것입니다. 즉 IV 1mpk 에서 하더라도 PO는 dose dependency가 있는 구간이라면 10mpk 와 비교해서 환산해 각 AUC를 구해 BA를 구합니다. 다시 말씀드리지만 언급하신 논문들은 주로 용출시험이나 BE test 쪽일 겁니다. Dose가 다르고 제형의 획기적인 변화로 BA값이 달라진 경우 우선 BA=AUCpo/AUCiv로 일반적으로 dose denpendency 가 있는 구간에서는 AUCiv는 Dose에 비례합니다. ( D/AUC= CL(Clearance) parameter 일정한 값이니까요.) 탁센 (250mg) 의 경우 기존 나프록센제제(375mg)와 비교할 경우 따라서 BA는 우선IV 100mpk 데이터 있다고 가정하고 100~375mpk 가 dose denpendency 가 있는 구간이라고 가정하며 D/AUCiv =CL 로 일정한 momoexponential decay 따른다고 가정할 시에 AUCpo250mpk/ AUCiv100mpk*2.5 : AUCpo375mpk/AUCiv100mpk*3.75 요렇게 BA값을 비교 계산하면 되겠네요. 그 밖에 아까 말씀 드린 임상자료, 독성자료, PK자료, TK자료 등을 첨부해야 하지만 몇가지 bridge test 퉁치는 경우도 많습니다. 지금 이 단락을 읽고 이해를 하신 분들은 저에게 컨설팅비를 주셔야합니다.주로 대학원 심화과정이나 제약회사 컨설팅 할 때 말씀 드리는 수준입니다. 제 CV 보내드릴테니 품의결제올려서 제계좌로 컨설팅비를~ 농담이구요. 컨설팅비 주시기 싫으시면 추천 한번으로 퉁치겠습니다. 일반인 분들은 패스해주세요. 특히 생리통같이 빠른 효과가 중요한 질환에서는 액상 제제가 아주 좋습니다.
