○ 영의정 서명선이 아뢰기를, “제주 목사 엄사만이 해를 넘기도록 장독(瘴毒)이 있는 바닷가에서 지내 병세가 위중해졌다고 누차 사장(辭狀)을 올렸습니다. 개차(改差: 교체하다)하는 것이 어떻겠습니까?” 하여, 그대로 따랐다. 조선왕조실록 정조 8년 (1784) 11월 3일 기록입니다. 조선사람들은 바닷가와 같은 습한 곳, 특히 덥고 습한 곳에서 장(瘴)이라고 하는 특수한 성질의 기운이 올라온다고 생각했어요. 이 기운은 사람의 몸에 침입해서 지속딜을 넣어서 점차 쇠약하게 만들고 끝내는 사망하게 만드는 무서운 놈이에요. 특히 우리 몸의 기운이 쇠약해져있을 때 장독에 노출되면 고열과 함께 크게 앓아서 죽을 수도 있었어요. 장독으로 인해 생기는 병, 장(瘴)이 온난다습한 기후로부터 생긴다는 것, 장병 유행지역이 대체로 이런 지역과 일치한다는 점 때문에 현대의 의료사 연구자들은 대체로 장병을 말라리아와 동일시하는 경향이 있어요. 이는 근거가 없는 게 아닌게 온난다습한 지역에서 모기가 번창할 확률이 높고, 그렇다면 동시에 말라리아에 감염될 확률이 높을 테니까요. 서양이름인 mal-aria도 직역하면 "나쁜 공기"인데, 말라리아 원충(原蟲) 발견 이전까진 물웅덩이 등에서 올라오는 기운이 말라리아의 직접적인 원인이라고 생각한 걸 보면 동양이나 서양이나 이 병의 원인이 대강 어디서 오는지 짐작은 하고 있었음을 알 수 있어요. 하지만 장(瘴)=말라리아는 아니었어요. 일제시대 당시의 기록을 살펴보면 조선의 말라리아의 유행지역은 해안/산간지역을 가리지 않았고 남/북을 가리지도 않았거든요. 모기가 있는 곳이라면 어디에나 있었고, 함흥이나 원산 처럼 추운 지역에도 있었지요. 특기할 만한 지역 분포 특징은 전라도 충청도 처럼 논농사가 많은 지역에서 말라리아 환자가 많았다는 거에요. 뭐... 당연한 결과지요. 나아가 조선사람들은 말라리아를 특칭하는 별도의 병명을 알고 있었어요. 학질(瘧疾). 학질은 장독/장병과 구분되는 명칭이므로 장독을 곧바로 말라리아로 대입해서 보는 관점은 설득력이 높지 않아요. 그래서 대체로 장병은 말라리아를 필두로한 열대성 풍토병을 통틀어 말한 게 아닐까하고 추측한답니다. 그렇다면 위의 실록의 기록처럼 해안가에서 장병 걸렸다는 기록이 조선시대 문서들로부터 수두룩 빽빽하게 보이는 건 왜때문일까요? 뭐 해안가 지역에서만 유행하는 지역 풍토병에 걸렸다거나 회를 먹고 배탈이 났다거나 하는 병리학적 접근을 해 볼 수도 있지만 제가 강력하게 미는 가설은 [병을 책으로 배워서...] 입니다. 장(瘴)은 중국에서 처음 쓰인 말이에요. 어디 삼국지연의에서 시작해봅시다..! 五月驅兵入不毛 오월에 병사 몰아 불모지에 들어감에 月明瀘水瘴煙高 밝은 달 밤, 노수에는 장독 안개 높아라 誓將雄略酬三顧 웅대한 계책으로 삼고의 은혜 갚고자 맹세했으니 豈憚征蠻七縱勞 어찌 남만을 정벌하여 일곱 번 놓아주는 수고를 꺼리랴 이게 공명이 맹획을 두 번짼가 세 번짼가 잡았다 놓아주고 나서 소개되는 시에요. 공명이 지었다고 하진 않고 후인(後人)이 지었다고만 하지요. 여기서 중요한 건 바로 장독이에요. 덥고 습한 남쪽 땅, 특히 강물에서 피어오르는 안개가 바로 장병의 직접적인 원인이라고 믿었어요. 이건 나관중의 관점이라기보단 장(瘴)이라는 단어가 생긴 이래로 거의 모든 한족 중국인들이 공유하던 인식이었어요. 이 단어는 서한-동한 즈음에 완성되었을 걸로 추정되는 한족 중국인들의 가장 오래된 사전인 설문해자(說文解字)에는 나오지 않는 단어에요. 삼국지의 영웅들이 활약할 당시엔 애초에 없던 말이라는 뜻이지요 (-_-;;). 그러다가 위진시대 즈음 해서 처음 용례가 등장하는데 그 이유는 서진이 망하고 동쪽으로 이사가면서 북방 출신 한족들이 대거 양자강 하류지역, 곧 강남(江南)지역으로 이주하면서 그지방 풍토병에 호되게 당했기 때문이래요.  저기 상하이 인근에 도로가 빽빽히 찬 곳 보이세요? 저기 상하이, 난징, 쑤저우, 항저우 인근을 대략 퉁쳐서 강남지방이라고 했는데 지금이야 중국에서 가장 부유하고 번성한 동네지만 진(晉)나라 당시엔 저긴 뭐... 시골이었지요. 또 지금도 각종 호수가 많이 보이지만 당시엔 수리작업이 제대로 안 되어있어서 정말 사방천지 늪지대였다고 해요. 늪지대가 많다면? 모기! 그래서 말라리아 유행지역이었을 것으로 추정되고, 주로 말라리아를 필두로 한 수인성(水因性) 질병에 북방출신 귀족들이 속속 쓰러지면서 그걸 퉁쳐서 장(瘴)이라고 이름붙이지 않았을까 하고 추정한답니다. 장(瘴)은 음을 나타내는 장(章)과 뜻을 나타내는 엄(疒)을 합해서 만든 형성자(形聲字)에요. 하지만 형성자라고해서 소리부문 글자의 뜻이 완전히 무시되진 않아요. 예를 들면 본성/성질을 나타내는 성(性)은 뜻을 나타내는 심(心)과 음을 나타내는 생(生)이 합쳐져서 만들어진 형성자지만 엄연히 "날 때부터 가지고 나오는 특질"이라는 의미에서 생(生)의 의미를 가지고 있거든요. 마찬가지로 장(瘴) 역시 음부분인 장(章)이 그냥 랜덤으로 배당된 건 아닐 거라고 추측할 수 있어요. 가장 유력한 추측은 장(障)의 의미가 있지 않을까 하는 거에요. 북방인이 남방으로 넘어올 때 장애물이 되는 질병이라는 거지요. 동진시기의 강남개척기에 이렇게 만들어진 신조어인 장(瘴)은 그 후로 한족 중국인의 활동무대가 남쪽으로 내려갈 때마다 그들과 함께 남쪽으로 이동했어요. 관우가 지켰다는 형주(荊州), 곧 오늘날의 호북(湖北)/호남(湖南)성 지역도 장병으로 유명했고, 공명이 평정했다는 남만지역인 사천(四川)성 남쪽, 귀주(貴州) 지역도 장병으로 유명했지요. 강서(江西)성 남부야 당연하고 남쪽 해안가의 복건(福建)성 역시 한족이 진출하면서 장병유행지역에 편입되었어요. 게중에 가장 장병으로 오랬동안 악명높았고 또 극악했던 곳은 영남(嶺南)지방, 오늘날의 광동(廣東)성/광서(廣西)성이었습니다.  저기 홍콩 인근 보이시죠? 저기가 광동성이고 그 왼쪽이 광서성이에요. 이 지역은 산맥 하나가 가로로 쭉 이어져서 위쪽 다른 지역들과 자연스런 경계를 이루고 있는데 그래서 한족의 진출도 꽤 늦었고 대량유입 역시 꽤 늦은 시기에 이루어졌어요. 산맥 사이사이로 난 고개를 통해 더 북쪽지역과 연결되는데 이런 고개가 다섯개 있다고 해서 오령(五嶺)이라고 하고 오령 이남지역이라고해서 별칭이 영남(嶺南)이에요. 우리나라의 영남지방도 그 이름을 여기서 따온 거구요. 하여간 이 지역이 아무래도 한족문화권에 늦게 편입되다보니까, 또 지리적으로 격절(隔絕)되어있다보니까 당나라 즈음부터는 그냥 장병과 관련된 오명을 덤태기 써버리게 되었어요. 북방인들이 오령을 넘기만 하면 바로 장병에 걸려서 열에 여덟은 죽는다느니, 반란을 진압하려고 군대를 보냈더니 5할이 장병으로 사망했다느니 하는 이야기를 심심찮게 볼 수 있지요. 송나라 때의 한 서적에서 증언하기를 당시 사인(士人)들은 광동에서 특히 장병으로 유명한 몇몇 지역을 대법장(大法場)이라고 불렀대요. 법장은 오늘날 말로 처형장 같은 건데 대법장이면..음.. 슈퍼처형장 같은 느낌? 상황이 이렇다보니 영남인들은 엄청난 차별에 시달렸어요. 영남지방에 임명되는 지방관들은 설령 부임지까지 온다고 해도 관사에 박혀서 나올 생각을 안했고 대개는 퇴임할 때까지 관사가 있는 도심 한가운데 밖으론 나가질 않았어요. 시골 군수나 현령 자리 같은 건 보낼 관리를 찾지 못해서 7~8년간 비어있는 일도 잦았구요. 중앙정부에선 영남지역 부임자에게 더 많은 급료와 승진기회를 약속했음에도 불구하고 임명장이 날아오면 울면서 "제가 노모와 처자식이 있는데..." 하는 통에 사람을 못보내서 그랬대요 -_-; 당나라 때 한 관리는 북베트남지역, 지금 위의 지도상에서 하노이 인근에 발령이 났는데 그걸 못가겠다고 거부하다 처형(!!)을 당하기도 했고, 다른 관리는 얌전히 광동지역에 부임했다가 장병으로 바로 사망하고, 시신을 수습하기위해 그 지역에 내려간 아들 역시 사망하고... 뭐 그런 처참한 이야기들이 많았어요. 남송 말 한 때 광동성의 성도인 광저우(廣州) 자사 정도로 임명되었던 류극장(劉克莊)이란 양반은 광동 바로 옆 지역, 복건성(지도상의 푸지엔 Fujian) 해안가 출신이었어요. 사실 지리적으로 영남지역과 별 다를바 없는 지역인데도 오령을 넘어 광저우로 들어가면서 "오령을 넘어 남하하니 북방인으로서 장병이 걱정이다" 와 같은 코멘트를 남겼어요. 아이쿠, 복건성 해안가가 북방이면 얼마나 북방이라고 광동과 남북을 따지다니. 여기서 이른바 "남방"과 "북방"이 기실 위도상의 고저 문제가 아니라 일종의 역사-문화적으로 구축된 가공의 관념임이 드러나요. 실제로 거기가 남쪽인가 아닌가, 더운가 아닌가, 습한가 아닌가는 사실 별 상관 없이 전혀 별개의 이유로 남/북이 갈린다는 거에요. 그렇다면 남/북을 가르는 기준은 뭐였을까요? 복건성은 당나라 정도 까지만해도 장병유행지역이자 "남방"이었어요. 송나라 들어서 경제가 성장하고 무엇보다도 [과거시험에서 뚜렷하게 우수한 성적을 내면서] 장병 유행지역에서 벗어나고 "북방"의 일부가 되었어요. 남송시기를 통틀어서 가장 많은 합격자를 배출한 지역이 바로 복건성이었으니 말 다했지요. 자, 과거시험에서 우수한 성적을 내면 남방도 북방이 된다...? 맞아요. 한족 엘리트 문화에 얼마나 코드를 맞췄느냐가 곧 남/북을 가르는 결정적인 기준이었어요. 달리 말하자면 한족 문화와 코드가 어긋나면 바로 남방, 장병(瘴病) 유행구역이라는 낙인을 받는다는 말이에요. 똑같은 병이어도 남방에서 걸리면 장병 걸렸다고 하고 북방에서 걸리면 다른 병, 예컨대 상한(傷寒)이라든가 학질이라든가 하는 다른 이름을 붙였을 거라고 추정해볼 수 있어요. 여기서 장병 담론의 인종적 요소가 드러나요. 장병은 단순히 남부지역의 풍토병을 가리키는 말, 말라리아를 가리키는 말이 아니라 남방지역에서 볼 수 있는 비-한족적 특성 전체를 가리키는 수식어로 기능했어요. 위에 삼국지연의에서 발췌한 시로 다시 돌아가보아요. 만(蠻)자가 보이시나요? 장(瘴)은 오랑캐 병이에요. [저] 오랑캐들이 [우리] 한족들에게 옮기는 무시무시한 병이지요. 1407년, 버마 원정을 떠났던 장홍이라는 명나라 장수는 이렇게 말했어요. "버마인들은 종종 음란한 여성을 훈련시켜 우리 병사들을 꾄다. 이들과 관계를 맺으면 반드시 병들어 죽는데, 그래서 이들을 인장(人瘴)이라고 부른다." 말도 안되는 이야긴데 이걸 사람들이 믿었어요. 그래서 그가 통솔하던 병사들은 이 지침을 명심해서 장병에 걸리지 않고 무사히 고향에 돌아갈 수 있었다라나 뭐라나. 광동과 광서지방 장병의 덤태기를 썼던 건 단순히 지역의 격절성 때문만은 아니었어요. 그 지역에 이민족이 엄청 살고 있었거든요. 지금도 광서성은 좡족 자치구(壯族自治區)잖아요? 광동성 역시 과거로 갈수록 묘족(苗族) 등 비한족 인구가 한족을 훨씬 상회했었을 것으로 추정된답니다. 한 가지 재미있는 사실은 사서와 개인기록등을 통틀어서 과거 전염병 발생과 유행사례를 역학(疫學)조사해보면 대부분의 전염병은 수도 및 번성한 대도시에서 발생된 걸로 나타난다는 거에요. 송나라의 경우는 개봉(開封)과 항주(杭州), 원나라의 경우는 북경(北京)과 항주, 명나라도 북경과 남경(南京)과 항주 등등. 문화적/경제적으로 가장 번화하고 가장 세련되고 가장 콧대 높았던 지역이 실은 병리적으론 가장 위험한 곳이었다는 거지요. 사실 당연한 게 이 도시들의 인구밀도는 당시 세계 최고 수준이었거든요. 남송시절 항주는 군사적인 이유로 사방이 산과 물로 막힌 비교적 작은 구역이었는데 이미 등록된 인구만 100만명이 넘었어요. 전염병 일어나기 딱 좋은 환경이지요. 광동/광서지역에서 최종 과거시험을 보기 위해 이런 도시들로 상경했었을 젊은이들은 아마 며칠이 지나기 전에 당장 풍토병에 걸려서 물갈이를 심하게 하거나 심지어 죽는 일도 있었을 거에요. 그렇지 않더라도 화려한 밤문화에 홀려서 기방에 들어가서 거사를 치렀다가 당장 무슨 병에 걸려서 고생을 크게 한 경우도 있었을 거에요. 하지만 우리는 모두 담론의 노예 아니겠어요? 깨끗한 북방, 병든 남방이라는 지배담론의 거대한 힘에 눌려서 남방인 역시 북방에 가서 풍토병에 걸려 고생할 수 있을 거라고는 상상조차 할 수 없었을 거에요. 광동 촌구석에서 올라온 애송이가 감히 제국의 수도 개봉의 기방에서 만난 천사 같은 여인이 인장(人瘴)일 거라고 상상할 수나 있었겠어요? 그래서, 기록이 없어요...;; 이 두 지역은 명나라 때를 즈음하여 장병의 오명을 벗게 돼요. 두 가지 이유가 있어요. 하나는 남송 말을 전후로 이 지역에 한족의 유입이 급증하면서 명나라 때부터 과거시험에서 유의미한 성적을 내기 시작했던 것, 다른 하나는 운남(雲南)지역이 명나라 영토에 들어오면서 장병구역이라는 덤태기를 쓰게 된 것. 폭탄 돌리기 같지요. 남이 뒤집어 쓸 때까지 내가 뒤집어 쓰는 것. 운남 지역은 이 때 장병지역이라는 이름으로 조리돌림을 심하게 당한 나머지 무려 지금까지도 장병이 있다고(?) 합니다. 무슨 말인가 하면, 대개 중국인들이 장병이라는 이름으로 뭉뚱그려 칭했던 말라리아의 원인과 치료법이 모두 밝혀졌음에도 불구하고 현지인들은 말라리아와는 별개로 장병에 걸리며(걸렸다고 주장하며) 자체적인 치료법을 사용하고 또 병을 피하기 위한 자체적인 노력을 기울인다고 해요. 이는 "질병"이란 인간이 인지하고, 이름 붙이고, 원인을 지목하고, 회피법과 요양법과 치료법을 만들어내고 공유할 때라야 비로소 성립되는 존재라는 인류학의 오래된 가르침의 한 실증사례라고 할 수 있어요. 우리가 그 병이 있다고 믿을 때, 그 병은 있어요. 자, 이제 글을 마무리하기 위해 조선으로 다시 돌아가볼께요. 글의 첫머리에 인용한 구절 뿐 아니라 실록, 문집을 막론하고 남방의 해안가와 장병을 연결해서 설명하는 구절은 그야말로 무수히 많아요. 이들은 천편일률적으로 바닷가에서 올라오는 습한 기운, 습한 바람을 장병의 원인으로 지목하지요. 하지만 어떤 증상을 인지해서 그걸 장병이라고 이름 붙이고 그 원인을 지목하고 회피법과 요양법과 치료법에 대해 해설하는, 장병에 관련된 그 모든 [말]들을 가만 들여다보면 이건 자생적으로 등장했다기보단 다 서적을 통해 수입된 거라는 의심을 지우기 어려워요. 다시 말해 광동/광서성의 기후/풍토와 장병을 연결시키는 한족 중국인 사가들의 기록을 보고, 남방으로 귀양가서 장병으로 어떤 고생을 하는지 세심하게 그려낸 소동파의 절절한 시를 읽고 그걸 그대로 배운 걸 거라고 추측해볼 수 있어요. 아, 덥고 습한 남방지역에 가서 걸리는 무서운 병을 장병이라고 하는구나, 내가 지금 거제도에 유배당해서 삭신이 뿌드드하고 기침이 나는 건 그냥 감기에 걸린 게 아니라 장병에 걸린거구나, 소동파가 하이난에서 걸렸다는 병이 이건가보다, 아 내가 제주목사를 맡고 나서 몸이 안 좋은 건 술을 많이 마셔서가 아니라 장병에 걸린거구나 하구요. 그런데 제가 왜 이런 글을 쓰고 있는 걸까요. 자꾸 공부하기가 싫고 딴짓만 하고 싶고 심기가 불안정한 걸 보니 장병에 걸렸나봐요. 어쩐지 오늘 비가 많이 와서 덥고 습하더라니.
