https://newrepublic.com/article/133008/cure-fear 머렐 킨트(Merel Kindt)는 암스테르담 대학의 임상심리학 교수입니다. 이 교수는 인간이 가진 공포스런 기억을 알약 하나로 잠재울 수 있다는 걸 발견했어요. 약의 효능은 거의 사기가 아닌가 싶을 정도로 놀랍습니다. 남아프리카의 요하네스버그에 휴가를 갔다가 강도를 맞이하고 심각한 불안증에 시달리던 환자가 있었어요. 이 환자는 클레버부인으로 요하네스버그의 호텔에서 곤히 자던 중 새벽 2시 쯤 인기척에 잠을 깼죠. 깨어 보니 왠 강도가 자신의 머리에 총을 겨누는 것이 아니겠어요. 순간 본능적으로 몸을 바짝 웅크리는 통에 옆에서 자던 남편도 깼죠. 강도는 현금을 요구하더니 낚아채서는 달아났습니다. 그 사건 이후 56세의 클레버부인은 정상적인 생활을 하기가 힘들었어요. 주변의 모든 것이 당시의 기억을 상기시켜서 낯선 남자만 보면 불안에 떨고 바람에 커튼이 날려도 잠을 이루지 못했죠. 그 사건 이후 더 고립되고 심약하게 변해버렸습니다. 심리치료를 거부하던 클레버부인은 일 년 반이 지나 신문을 보고 머렐 교수의 연구소를 찾아갑니다. 먼저 머렐 교수가 아닌 심리치료보조사들이 일련의 질문들 했고 클레버부인은 심적 외상 후 스트레즈 장애증상 등 자신의 상태에 대해 소상히 늘어놓죠. 삼 주 후 클레버부인은 머렐 교수와 만나게 되는데요. 보통 심리치료사라고 하면 친절하고 상냥하게 환자를 맞이할 것이라 기대하지만 머렐 교수는 단도직입적이었죠. 교수는 클레버부인으로 하여금 당시의 공포스럽던 기억을 최대한 헤집고 다니게 만들더니 대뜸 '치료법이란 없다'고 합니다. 클레버부인이 눈물을 훔쳐도 머렐 교수의 답변은 '당신을 도울 수 있는 사람은 아무도 없다'는 거였어요. 약 15분간의, 상담이라기보다 심문에 가까웠던 대화를 마치고 머렐교수는 클레버부인에게 알약을 하나 내밀었고 클레버부인은 절망에 싸인 채 주는 약을 삼켰죠. 그 날 밤 12시간의 잠을 청하고 다음날 기상한 클레버부인은 자신의 기억에 변화가 왔다는 걸 단박에 느낄 수 있었어요. 요하네스버그 호텔에서의 끔찍한 기억을 전과 변함없이 되새길 수 있긴 했으나 더 이상 공포스럽지가 않더래요. 한결 가벼워진 기분으로 그 때를 상기할 수 있었다고 합니다. 공포는 완전히 사라졌대요.  이 약은 프로프라놀올이라는 약으로 심장질환을 치료하는 약이라고 합니다. 그닥 비싸지도 않고 흔한 약이라고 하네요. 머렐 교수는 공포와 기억이라는 연구영역에 헌신해 왔는데 아마 10년 내, 외상 후 스트레스 장애나, 공포증, 불안장애증을 획기적으로 치료할 날이 오지 않을까 믿고 있다고요. 실험에 참여한 환자들이 치료되는 과정을 보며 매번 놀라움을 금치 못한대요. 마치 과학이 아니라 요술 같다는군요. 심리학에서 치료제라는 건 썩 어울리는 용어가 아니지만 머렐교수의 경우 확실한 치료제를 개발한 것 같습니다. 공포란 건 무조건 치유되고 없어져야 할 것은 아니에요. 때로 공포는 우리의 생존에 도움이 됩니다. 인간이 위험에 처하면 아드레날린이 과다분비되고 심장을 널뛰기를 하며 어떤 상황에서 맞서 싸워야 할지 도망가야 할지 민첩하게 판단하도록 자극을 줍니다. 그치만 인간에 해가 되는 공포도 있죠. PTSD(외상 후 스트레스 장애)나, 각종 포비아, 불안장애 등이 그것인데요. 환자 중에는 자국을 떠날 때마다 패닉 발작을 일으키는 경찰관도 있었고 친어머니의 자살을 감당하지 못해 괴로워하는 환자도 있었고 고소공포증이나 거미공포증, 뱀공포증 등을 가진 환자들도 있었어요. 기존의 임상심리학에서 이들을 치료하기 위해 시도한 방법은 노출요법이었어요. 이를테면 뱀에 노출시켜 뱀이 위험한 동물이 아니라는 사실을 일깨움으로써 뱀에 대한 공포를 가시게 하는 거죠. 노출요법은 치료기간이 길고 감정적으로 사람을 지치게 만들기도 하고 시간이 지나 재발하기도 한다는 단점이 있어요. PTSD같은 경우 치료상태가 겨우 30분밖에 유지되지 않는대요. 대신 머렐교수의 치료법은 치료기간도 훨씬 짧고 효과가 거의 영구적이며 항우울처방도 필요가 없다고 합니다. 에릭이라는 환자는 뱀공포증을 치료하기 위해 머렐교수를 방문했다가 치료상태를 확인하기 위해 다음 날 교수의 연구소로 다시 찾아갔는데 뱀을 맨손으로 만지기까지 했다고 합니다. 하룻만에 공포가 싹 가셨더래요. 파충류가 득시글한 곳에 가서 뱀에 둘러싸여도 하나도 무섭지가 않더라는군요. 에릭은 치료를 시도하기 전 생각하길 뱀공포증이 순식간에 사라지면 일생일대의 정신승리일 것이 분명하다고 생각했는데 막상 치료가 되니 그냥 별 생각이 없더래요. 앗 배고프다. 잠만요. 나머지는 좀 있다 옮길게요. 간질간질 ㅋㅋ 식빵 구워먹고 에너지 충전. 1920년, 미국심리학자 존 왓슨은 포비아가 부정적인 경험을 통해 학습되는 것이라고 했지요. 이 가설을 증명하기 위한 실험이 ‘Little Albert’라는 실험이었어요. 어린 아기 알버트에게 하얀 쥐를 보여주며 동시에 철봉을 무섭게 내리치자 하얀 쥐만 봐도 무서워하고 털이 난 물건만 봐도 무서워했어요. 도의적으로 잘못된 실험이었지만 공포가 경험에 의해 학습되는 심리라는 중요한 학문적 발견을 이루어 냈어요. 그 후 심리학자들은 왓슨의 발견을 정신의학에 적용 해 환자들에게 긍정적인 사고를 고무시켜 내면의 공포를 희석시키려고 노력했어요. 임상심리 박사과정을 밟던 1992년, 머렐은 노출요법이 불안장애를 치료하는 최선의 방법이라고 배우던 중이었죠. 고소공포증이 있는 이를 높은 곳에 데려가 적응을 시키면 결국 높은 곳이 나를 해치는 곳이 아니란 걸 배우게 되고 이런 경험의 축적이 기존의 공포를 완화시켜 줄 것이란 것이 노출요법의 내용이고요. 그치만 노출요법도 한계가 있어요. 공포의 기억 속에는 어떤 규칙같은 것이 있어요. 높은 발코니가 더 이상 무섭지 않음을 배우게 된 환자는 다른 높은 장소는 여전히 무서워합니다. 치료가 끝난 후 공포는 언제든 환자를 압도할 수 있고 그럼 다시 패닉에 휩싸이게 되죠. 공포의 기억이란 건 뇌관이 제거되지 않은 채 숨겨져 있는 폭탄과 같아요. 머렐은 노출요법이 왜 어떤 경우에는 효과가 있으면서 어떤 경우엔 또 효과가 없는 건지가 궁금했어요. 공포의 기억을 한꺼번에 가시게 할 수 있는 방법은 없을까 의문을 품었죠. 대부분의 과학자들은 그건 불가능하다고 생각했어요. 한 번 꽂힌 기억은 평생 간다고 믿었죠. 1960년대에 과학자들은 새로운 기억이 어떻게 탄생하게 되는지 정확한 경로를 찾아내려 했어요. 실험쥐에 뇌 속에서 단백질합성을 억제하는 약을 투약했죠. 종소리와 전기충격을 학습시켰더니 당시에는 종이 울릴 때마다 공포에 떨었는데 신기하게도 다음 날이 되자 종을 울려도 무서워하지 않았어요. 공포가 학습되지 않은 거예요. 약물이 기억을 형성하는 걸 방해했거든요. 이 실험을 통해 배운 사실은 장기기억이 가능하려면 단백질합성이 일어나야 한다는 거였어요. 단백질합성이란 새로운 기억이 뇌속의 세포조직을 살짝 바꾸는 작업이에요. 이 과정을 ‘기억통합’이라고 합니다. 과학자들은 이 기억통합(경험과 기억의 저장과정)이 학습이 일어난 수 시간 내에 각각의 기억 속에서 딱 한 번 일어나는 거라고 믿었어요. 다른 종류의 기억은 뇌의 다른 부분에서 통합하는데요. 선명한 일생일대의 기억통합은 뇌의 해마라는 데서 일어나고 공포와 같은 갖가지 감정의 기억통합은 소뇌 편도에서 일어나서 싸우냐 도망가냐를 결정하게 합니다. 불안장애는 소뇌 편도에서 과도하게 일어난 기억통합에 의해 생긴다고 과학자들은 당시 주장했습니다. 쥐가 어떤 기억을 학습하자마자 뇌속의 스트레스 호르몬수치를 바꿔주면 쥐의 기억력을 강화시키거나 약화시킬 수 있다는 걸 알게 됐죠. 예컨데, 아드레날린이 과다분비되면 쥐는 미로를 더 잘 기억하게 됩니다. 아드레날린 억제제를 투약하면 미로를 잘 기억하지 못했어요. 인간도 스트레스로 인해 아드레날린이 과다분비되면 당시를 더 생생하게 기억합니다. 기억통합(경험들과 단기기억들을 통합, 정리해서 필요한 기억들만 저장시키는 작업)이란 것 때문에 한 번 각인된 기억인 이상 노출요법 해봤자 별 효과가 없다는 결론에 이르게 되니(당시엔 그렇게 믿음) 심리학자들은 그냥 그러한 기억을 간직한 채 다른 긍정적인 기억을 고무시켜 치료하려고 애썼죠. 이 치료법에서 공포의 기억이 사라지는 일은 없습니다. 어떤 의사들은 어떤 치명적인 경험의 순간이 일어나고 몇 시간 지나지 않은 시점, 즉 기억통합이 일어나기 전에 뇌의 호르몬을 조절하는 약물을 투입하면 괴로운 기억에 묶여버리지 않을 것이라 생각, 어떤 응급실병동에서 어떤 환자들에게는 약물치료를 하고 어떤 환자들에게는 위약(플라시보)처방을 했더니 삼 개월 후, 위약을 적용한 환자의 거의 반띵은 손에 땀이 난다든지 심박수가 올라가는 등의 외상 후 스트레스 장애 전조증상을 보였고 호르몬 조절약물을 투약한 환자군에서는 아무도 외상 후 스트레스 장애증상을 보이지 않았대요. 이 때 사용한 약이 프로프라놀올입니다. 그런데 이런 치료는 도의적으로 문제가 될 수 있습니다. 트라우마를 겪고 나서 PTSD증상으로 악화되는 환자군은 보통 10-30프로에 불과한데 이 중 누가 증상이 악화되고 누가 악화되지 않을 것인지 알 수가 없어요. PTSD를 확인하려면 한 달은 족히 기다려봐야 합니다. 트라우마의 경험 후 몇 주 동안 심리적 희생자가 되는 건 보편적인 겁니다. 누가 심리적으로 악화될 지 알 수 없는 상황에서 약물치료부터 하면 쓸데없이 기억을 인위조작하는 것이 되므로 꺼림칙하죠. 그건 내 고유의 기억일 수가 없어요. 그러다 2000년에 새로운 사실이 발견됩니다. 기존에 한 번 꽂힌(기억통합된) 기억은 평생을 간다고 이해하고 있었으나 쥐를 실험하다가 우연히 기억이란 건 '재'통합된다는 것을 알게 되었어요. 이게 무슨 말이냐면 한 번 각인된 기억이 죽 가는 것이 아니고 매번 회복되는 거란 거예요. 이런 실험으로 설명이 가능합니다. 쥐에게 특정 소리와 전기충격을 학습시킵니다. 그리고 24시간 쉬게 합니다. 그러면 기억통합이 일어나겠죠. 즉, 기억이 저장되겠죠. 그리고 나서 특정 소리를 들려주어 어제의 학습기억을 되살려 줌과 동시에 뇌 속에서 단백질합성을 억제토록 약물을 투여합니다. 그 다음 날 같은 소리를 들려주어도 쥐는 무서워하지 않았어요. 이로서 알 수 있는 사실은 오래된 기억은 (꽂히면 쭉 가는 것이 아니라) 기억이 재생될 때마다 이도 저도 될 수 있는 유연한 상태에 놓여 있게 된다는 거였어요. 미래에 다시 등장하기 위해서는 단백질합성을 필요로 하구요. 이 새로운 사실을 출판하기가 주저스러웠던 것이 기존의 '기억통합론'을 뒤집기 때문이죠. 이 실험내용을 Nader라는 사람이 네이쳐지에 싣게 되고 임상심리학은 새로운 국면을 맞이하게 됩니다. Nador의 실험발표 후 연구재연를 통해 여러 많은 동물군에서 기억은 재통합(단백질합성의 힘으로 다시 저장됨) 되는 것임이 연달아 입증되었죠. 2003년에 머렐교수가 프로프라놀올을 가지고 쥐실험을 했더니 새로운 공포의 기억이 과도하게 통합, 저장되는 것을 막는 데 효과가 좋았대요. 그래서 이 약이 과연 오랜 기억이 재통합, 저장되는 것도 막을 수 있을까 생각해 봤대요. 2008년에 학생들을 대상으로 삼일 간 실험을 하기도 했죠. 첫쨋날, 세 그룹의 학생들을 컴퓨터 앞에 앉혀 놓고 거미사진을 보여줄 때마다 기분나쁜 전기자극을 줬어요. 둘쨋날, 두 그룹에는 프로프라놀올을 처방해주고 나머지 그룹은 위약(플라시보)처방을 했어요. 그리고 프로프라놀올 그룹 하나와 위약그룹에 거미사진을 한 번 보여줘서 어제의 기억을 되살렸어요. 결과는 너무도 명백했죠. 프로프라놀올을 처방받고 기억을 재생시켜준 그룹은 24시간 후 거의 무반응이었어요. 30일 후에도 무반응이었죠. 전기충격에 대한 기억도 무뎠다고 합니다. 2013년에는 거미공포증을 가진 이들을 대상으로 실험을 했어요. 유리관에 있는 (크고 털 부숭부숭한) 타란툴라거미를 보여주고 2분 후에 프로프라놀올을 복용하게 한 뒤 며칠 후 다시 불러 거미를 보여주었더니 전에는 쳐다보지도 못하던 이들이 가까이 가기도 하고 어떤 이는 손가락으로 거미털을 만져보고 그러더라네요.
