안녕하세요.일전에 아토피환자로서 AMA를 진행한 적 있는 에스와이에르입니다. ( https://new.redtea.kr/?b=28&n=1157 ) 의료넷이라고도 불리는 홍차넷인만큼 혹여나 답변해주실 수 있는 분이 계실지...해서 올려봅니당. 바쁘신 분들은 아래 [요약및 질문]만 읽어주셔도 됩니다~! 듀피젠트프리필드주300밀리그램(두필루맙,유전자재조합)는 국소치료제 및 기타 치료방법으로 치료가 어려운 중증 이상의 성인환자의 치료에 쓰이는 주사제라고 알고있습니다.최초 600mg 투여 이후, 2주에 한번 300mg씩 투여하는 방식으로 현재 대학병원에서 전문의의 처방 아래 투여가 가능합니다. 넷상에서 투여받으신 분들의 의견이나 임상후기등을 보면 결막염과 같은 일부 부작용을 제외하고는 획기적으로 치료가 되었다고 말씀하시는 분들이 계신데요.다만 2주마다 즉 지속적으로 투여받아야한다고 하네요. 여기서 문제가 발생하는데, 바로 이 듀피젠트의 300mg 주사당 가격이 96만원정도이고, 비보험이라는 점인데요. 따라서 2주에 한 번씩 꾸준히 맞으면 일 년(52주) => 96만원*26 = 2496만원이라는 가격을 환자가 부담해야합니다. 이는 저소득층 아니, 사실상 대다수의 환자가 부담할 수 없는 가격입니다. 이를 해결하기 위해 현재 환우들과 의사분들, 국회의원분들이 최근에 나서서 보험품목등재 촉구시위등을 개최했는데요. (디시인사이드'아토피'마이너갤러리 - https://gall.dcinside.com/mgallery/board/view/?id=atopic&no=20645&page=1) 보건복지부,심평원,사노피(제약사) 등에서 현재 협상중이라고 들었습니다. 개인적으로는 대학병원 및 병무청에서 중등도이상의 중증 아토피성피부염 진단을 받았고 5급 판정을 받았습니다. 또한 현재 통원하고 있는 동네의원에서 매 주 항히스타민제와 면역억제치료제를 처방받고 있으며 여름처럼 심한 경우 주사제를 처방받고 있는데 의사선생님께서는 대학병원에 가보라고 계속 말씀하셔서 다음 주 한양대병원에 치료를 예약해둔 상황입니다. 다만 일전에도 연대병원등 주요병원에 갔을 때(아마 그때는 듀피젠트가 활성화되지 않았을때라..) 결국 동네병원가 크게 다르지 않은 치료를 처방받고 했던 기억이 있어서 이번에 병원에 내원하더라도 듀피젠트같은 것을 처방받지 않으면 다를 것이 없을 것 같습니다... 그래서, 조금 더 사실관계를 파악하고 공부한 뒤에 직접 행동에 나서보려고 합니다. 저는 건선으로 휴미라(아달리무맙. 이것도 유전자 재조합입니다)를 주사받고 있고 2주에 1대를 맞습니다
결국 본인부담이 적어지는 중증환자를 어디로 설정할지, 어떤 병의 어떤 약에 본인부담을 줄일지는 순전히 정책에 달린 문제라 보입니다
한정된 자원으로 어느 병에 어느 정도까지 본인부담금을 줄여줄 것이냐의 문제이기 때문에

산정특례 대상이 되지 않았다면 아달리무맙을 제 돈 주고는 절대 안맞았을 것이고 그럼 건보 재정을 제가 이렇게 많이 쓸 일은 절대 없었겠죠...
한정된 자원으로 얼마나 많은 이들에게 어디까지 혜택을 주어야 하느냐. 참 어려운 문제인 것 같아요
