이번 기준 변경에 가장 큰 전기를 가져다 준 연구는 SPRINT trial이라고 해서
미 국립보건원 NIH의 funding을 받아 진행한 연구이므로 특정회사에 치우친 결과라고 하기는 어렵겠지만
소송과 더불어 로비의 나라 미국이다보니 그 예산을 집행하게 만든 것 역시 제약회사의 로비라는 이야기도 있긴 합니다.
SPRINT가 진행되기 전 있었던 여러 연구에서 목표 혈압을 더 낮추면 합병증 발생 위험이 줄어든다는
'The lower is the better'를 시사하는 경향이 관찰된 바 있었고,
이에 120mmHg 미만으로 조절하는 군과 140mmHg 미만으로 조절하는 군으로 나누어 Randomized controlled trial을
미국 102개 의료기관을 방문하는 환자를 대상으로 진행한 결과 더 낮게 조절한 군에서
사망, 심혈관 질환, 뇌경색, 심부전 등의 위험이 줄어들면서 혈압약의 심각한 부작용은 두드러지게 증가하지 않았기 때문에
이것을 근거로 몇몇 추가적인 연구와 분석이 이루어진 결과 고혈압 기준과 치료 목표를 모두 수정하기에 이른 것입니다.
국내에서도 기준 변경 이후에 여러 언론에서 의학전문기자나 교수님들의 견해를 실은 기사가 나왔고
뉴스 검색에서 고혈압으로 치면 간단한 소개를 하는 글을 많이 만나보실 수 있겠습니다만
연구결과 자체를 한글로 정리한 자료를 찾기는 쉽지 않을 듯 합니다.
우리나라에 적용될 지는 아직 모르겠습니다.
한국인 대상 연구가 진행중인 것으로는 알고 있는데, 내년 춘/추계학회 정도에는 발표하지 않을까요?
