청와대에서 서카딘서방정을 무려 600정이나 구입했네요. 서카딘정은 멜라토닌 (Melatonin) 2mg 로 수면보조제로 사용되는데 과연 누가 사용했을까요? 불면증에 시달리고 잠 못 드는 청와대 인물들이 많나 봅니다. 벤조디아제핀계열 진정-수면제인 자낙스 (Alprazolam) 도 300정 직접 구입했네요. 서카딘은 향정신성의약품이 아니지만 자낙스는 향정신성의약품으로 분류되어 엄격하게 관리되는 약물입니다.  불면 (Insomnia) 불면은 삶의 질을 떨어뜨립니다. 심지어 불면증에 시달리는 사람은 정상인들에 비해 일상생활에서 사고사 확률이 2.5~4.5배 정도 높다고 합니다. 몽롱하니 당연히 집중력과 주의력이 떨어져 사고 확률이 높을 것 같아요. 너무나도 잠을 자고 싶은데 자지 못하는 고통은 겪어보지 않는 사람은 모른다고 하네요. 물론 단순한 수면장애는 누구에게나 나타날 수 있습니다. 스트레스를 많이 받아 며칠 동안 잠을 편히 자지 못한 경험은 누구나 있을 거에요. 수면장애가 보통 주 3회 이상 발생하고 3개월 이상 지속된다면 불면증으로 의심할 수 있습니다. 또 우울증과 불안증을 가지고 있는 사람은 불면증 또한 가지고 있을 확률이 높습니다. 때에 따라 불안 -> 불면 -> 우울 테크를 타기도 합니다. 대뇌피질의 각성/수면에 관여하는 대략 6개 이상의 뇌신경센터가 있는 것으로 알려져 있습니다.  이런 각성/수면 시스템은 사람이 잠들고 깨는 것에 대한 스위치역할을 하기도 하고 일종의 양팔저울이나 시소와 같은 시스템으로 한쪽으로 기울 경우 각성/수면이 일어나기도 합니다. 각성에 관여하는 신경전달물질은 Norepinephrine, 세로토닌 (Serotonin), 히스타민 (5-HT, Histamine), 오렉신 (Orexin) 등 이고 수면에 관여하는 신경전달물질은 GABA, Galanin, 멜라토인 (Melatonin) 등으로 이들 신경전달물질을 매개로 각성/수면 시스템이 조절됩니다.  A - 각성 (Awake) B - 수면 (Sleep) C - 기면증 (Narcolepsy) - 기면증과 Modafinil (왜 감기약을 먹으면 졸릴까?) http://pgr21.com/?b=8&n=67195 원래 이 시스템은 밤낮은 번갈아가면서 작동하면서 수면/각성 일어나는데 이들이 밤에 제대로 작동하지 않으면 수면에 문제가 생기게 됩니다. 특히 밤낮으로 각성에 관여하는 뇌신경센터들이 꺼지지 않고 대뇌각성 작용을 계속할 경우 불면증이 일어납니다. 경우에 따라 수면에 결정적인 역할을 하는 GABA 관련 신경시스템인 VLPO (Ventrolateral Preoptic Nucleus) 라는 뇌신경센터가 꺼져 버려 불면증이 발생하기도 합니다. 원래는 우리가 잠에서 깨어나면 각성 뇌신경센터들이 VLPO를 끄고 잠잘 준비가 되면 VLPO가 다시 켜져 잠을 자게 되는데 이 VLPO 센터의 스위치가 제대로 작동하지 않게 되어 불면증에 빠지기도 합니다. 불면증 치료제 약물 치료는 전체 치료의 일부로 일시적으로 잠을 자게 만드는 약물이라서 뇌에 작용하는 약물답게 약물에 따라 내성과 의존성도 발생합니다. 스틸녹스 (Zolpidem)   스틸녹스-졸피뎀은 매우 안전한 약으로 많이 사용되는 약물입니다. 역시 필요시에만 사용하고 단기간에만 사용할 것을 권장하니다. 또한 마음대로 본인의 용량을 늘리지 말아야하면 술과 함께 복용하는 것을 피해야합니다. 사람에 따라 몽유병현상이 일어나기도 하는데 이때문에 많은 사고가 발생하기도 했습니다. 벤조디아제핀 (Benzodiazepine) 계 약물 - 항불안제로 사용되는 바리움 (Diazepam), 아티반 (Lorazepam), 할시온 (Triazolam), 자낙스 (Alprazolam) 등도 수면제로 사용할 수 있습니다. 기본적으로 진정-수면제 (Sedative-Hypnotics) 들이라서 용량에 따라 항불안, 진정, 수면 등에 사용할 수 있습니다. (1.불안해소 > 2. 진정 > 3. 수면 > 4. 전신마취 > 5. 혼수 > 6. 사망 ) 특히 벤조디아제핀 (Benzodiazepine) 계 약물들은 onset time과 반감기가 약물마다 차이가 커서 환자에게 맞는 약물을 선택하여 처방투약해야 하고 노인들에게 투여할 때는 신중한 관리가 필요합니다. ( 불안과 향정신성의약품 http://pgr21.com/?b=8&n=68694 ) 알콜과 함께 복용시 위험하고 부작용 확률 (다음날 아침에도 졸림, 어지러움, 피곤, 숙취, 인지능력 저하, 약물성 치매 등) 이 높아집니다. 수면유도제 항히스타민제인 Diphenhydramine이 사용되는데 14일 이내로 복용하길 권장합니다. 장기 복용시 효과가 떨어지는 것으로 알려져있습니다. 항히스타민제를 복용하면 Histamine 분비가 억제되고 Orexin시스템도 활성화되지 않으니 졸음이 발생하는 것으로 알려져 있습니다. 술 술은 먹으면 단기적으로 수면에 도움이 되지만 술에 반복적으로 노출되고 술을 많이 먹게 되면 REM 수면이 증가하여 REM Rebound가 발생해 자고나서도 불쾌한 느낌이 나타나는 등 숙취 등을 발생시킵니다. 그 밖에 프로포폴처럼 수면-마취제를 주사제로 사용하여 직접 잠을 재우기도 하고 멜로토닌, 카모마일, 길초근 같은 약물이나 생약제제가 사용되기도 합니다. 비약물적 요법 잠이란게 역시 뇌의 작용이고 심리적인 요인이 커서 비약물적인 요법도 중요합니다. 1. 침실을 어둡고 조용하게 하여 쉽게 잘 수 있는 환경을 만든다. 2. 낮잠을 피하고 일정한 시간에 잠에 들고 일어날 수 있도록 노력한다. 3. 부드러운 음악을 들으며 몸을 이완시켜 잠을 청한다. 4. 자기 직전 격렬한 운동을 삼가하고 가벼운 스트레칭이나 요가를 통해 몸을 이완시킨다. 5. 자기 직전 음식을 먹지 않는다. 6. 침대에 누워 일을 하지 않는다. 피부시술을 위해 수면-마취제 주사제를 자주 사용하면서 잠을 잤고 향정신성의약품인 진정-수면제를 전문가의 관리없이 무분별하게 계속 사용해왔다면 더더욱 정상적인 상황에서 잠을 자지 못했을 거고 더더욱 약물에 의존하게 되었을 것 같습니다. 청와대에서 잠 못 들고 불안에 떠는 사람들은 이제 약물에 의존하지 말고 그 자리에서 내려오면 저절로 불면과 불안에서 벗어날 수 있지 않을까 합니다. 미국에서나 다른 아시아 국가에서는 건강보조식품으로 규정해서 약국이나 GNC같은 건강보조식품 판배소에서 판매하고 있습니다.
가격도 훨씬 저렴하구요.
한국은 멜라토닌은 아예 수입금지였다가 서카딘이라는 서방형 제제가 나오면서 전문의약품에 비급여약품으로 출시했죠.
한 알에 대략 1000원에 판매되고 있습니다.
그런데, 미국에서는 2mg도 아닌 5mg, 10mg을 팔고 있으며 5mg 180알에 10-20달러 수준에서 팔고 있습니다.
