프로포폴 (Propofol)   유독 아픈 주사들이 있죠? 우리 체액과 같은 농도인 등장액 (보통 생리식염수, Saline) 에 녹지 않아 다른 형태로 만든 약물을 주사제로 맞을 경우 상당히 아픕니다. 프로포폴도 난용성 물질로서 생리식염수같은 등장액에 녹지 않아 정맥주사 (Intra-Venus, IV) 로 사용하기 힘든 편입니다. 어쩔 수 없이 Lipuro, 레시틴, 대두유 같은 용해보조제를 사용하거나 아예 현탁액 (Microemulsion, Liqiudemulsion, 우유빛 emulsion이라 우유주사라고도 불림) 으로 만들어 정맥주사로 사용합니다. 그래서 프로포폴을 맞으면 맞는 부위가 상당히 아파서 국소마취제인 리도케인을 소량 함께 투여하기도 한다고 하네요. 또 프로포폴은 정맥주사로 투여하는 경우 Onset time 이 매우 빨라 30초이내에 효과가 발휘되는 매우 뛰어난 약물입니다. 또 1st Phase의 반감기가 0.5~1시간으로 매우 짧고 최소유효혈중농도가 높은 편이라서 약물 중단 후 10 여분 이내에 마취가 풀리는 등 약물효과가 빠르게 사라져 마취제로서는 장점이 매우 많은 약물입니다. 하지만 마취제로서 효과를 지속시키기 위해서는 IV Bolus, IV Infusion를 함께 사용하거나 다른 마취제와 병용하기도 합니다. IV Bolus, IV Infusion  앞서 소개한 것처럼 반감기가 매우 짧은 프로포폴은 작용시간을 늘리기 위해 정맥주사로 IV Bolus, IV Infusion를 병행하여 사용하기도 합니다. 프로포폴의 특징상 정맥주사 한방 (IV Bolus) 후 작용시간 (Duration Time) 이 5~10분으로 매우 짧아 금방 마취가 풀리므로 IV Infusion 를 병행하여 마취시간을 늘립니다. IV Infusion에 따라 프로포폴의 작용시간은 한없이 늘릴 수 있습니다. 보통 용량은 나이나 각종질환의 유무, 상습투약정도에 따라 증감이 가능한데 일반적으로는 IV Bolus Dose 2~2.5 mg/kg (매우 아프므로, 수십초 간격으로 나누어서 투여) IV Infusion Dose 6~12 mg/kg/hr (Infusion Pump를 통해 일정한 속도로 지속적으로 투여) 를 사용합니다. 이와 같이 프로포폴을 IV Bolus, IV Infusion를 함께 사용할 경우 IV Bolus 투약하여 30초이내에 작용이 나타나고 IV Infusion으로 계속 약물을 투약하여 오랜 시간동안 약물효과를 유지시킬 수 있습니다. 역시 IV infusion 끝내면 곧바로 10 여분 이내에 약물효과가 사라지는게 일반적입니다. 효과가 나타나는 최소농도/ 부작용이 나타나는 최소농도의 사이를 Therapeutic range 라고 했죠? 프로포폴도 Therapeutic range 좁은 편에 속하는 약물이라서 일반적인 사용자는 모르지만 상습투약자나 위험요소가 있는 환자들은 TDM (Therapeutic Drug Monitoring) 을 해주면 좋을 것 같은데 실제 그렇지 않는 것 같습니다. 불법 상습사용자들이 많으니까요. 그래서 마이클잭슨이 이 프로포폴 상습복용으로 사망하고 말았죠. 2013년도에 문제가 되었던 프로포폴 남용으로 기소된 연예인들이 몇명이 있었는데 2011년부터 2013년까지 2년동안 A양은 185회, B양은 111회, C양은 95회 프로포폴을 투약 받았습니다. 이렇게 상습투약자라면 아마도 주1회이상 투약하는 것으로 여겨집니다. 이들은 상습투약으로 기소되었고 D양은 42회로 상대적으로 적어 약식기소되었습니다. (가수 휘성도 억울하게 연루되었으나 10회 정도로 명확히 의료용으로 사용되었음을 알 수 있습니다.) 일반적으로 프로포폴을 사용하면 잠을 푹 잘 수 있고 피로가 싹 가시며 기분도 상쾌해진다고 알려져 있어 바로 이때문에 잠을 많이 못자는 연예인들이 이 프로포폴에 쉽게 중독된다고 알려져 있습니다. 프로포폴의 사용이 문제가 아니라 상습사용자들은 의료용이 아닌 자신의 욕구를 만족시키기 위해 사용하고 상습투약으로 중독현상이 나타나기 때문에 문제입니다. 따라서 이에 대한 규제를 확실히 해야합니다. 또 남용의 끝은 마이클잭슨처럼 사망에 이를 수도 있기 때문에 국가가 나서서 관리해야하는 약물이기도 합니다.  위의 표와 같이 프로포폴을 짧은 간격으로 맞은 상습투약자들이나 긴시간동안 프로포폴 (IV Infusion) 를 사용한 투약자들은 약물을 중단한 후에 약물에서 깨어나는데 일반 투약자들보다 더 시간이 오래 걸립니다.  또 약물의 짧게 사용할 경우 혈중농도에서 빠르게 소실되어 별 문제가 없으나 상습투약하거나 오랜시간 프로포폴을 맞은 투약자들은 약물이 혈중에서 소실되었다고하나 실제로는 몸의 다양한 조직에 분포되었기 때문에 1시간 미만인 1st Phase의 반감기와 상관없이 2nd Phase의 반감기는 매우 길어 몸밖으로 프로포폴이 전부 빠져나가는데는 시간이 아주 오래 걸립니다. 이런 이유로 마이클잭슨처럼 프로포폴 상습투약자들은 부작용 위험성이 더 높습니다. 프로포폴은 짧은 작용시간과 반감기가 장점으로 발휘되는 마취제로 사용될 때 다른 마취제들에 비해 우수하고 상대적으로 적은 부작용으로 매우 안전한 약이지만 이는 사용빈도가 적고 짧게 사용하는 일반투약자들에 해당하는 것이고 상습적으로 프로포폴을 맞거나 프로포폴을 매우 긴시간동안 정맥주사 ( IV Infusion) 로 사용한 사용자들은 몸 안에 프로포폴이 축적되어 일반 사용자들에 비해 체내 반감기가 길어져 실제 프로포폴이 체내에서 제거되는데 오랜 시간이 걸려 약물 부작용의 위험성이 높아집니다. 그 밖에 프로포폴과 강력한 합성마약인 펜타닐과 함께 투약되기도 하고 다른 모르핀계 약물과 함께 사용되기도 합니다. 이런 중독성이 큰 약물을 의료용도로 사용하지 않고 상습적으로 투약 받는 것 자체가 범죄입니다. 이게 상식인가? 이를 관리해주는 의사는 뭔가요? 이게 나라인가요?
