옆동네 유머게시판에 유경험자의 출산 이야기가 아주 실감나게 올라왔네요. 콜 더 미드와이프라는 BBC드라마가 있는데 이게 책으로도 나왔습니다. 조산사가 주인공인데 출산 장면을 묘사한 부분이 인상적이어서 메모해 뒀던걸 옮겨봅니다. 처음 읽었을땐 긴장감이 느껴졌었는데 옆동네 경험담을 읽고 다시 보니 많이 건조하네요. 픽션과 논픽션의 차이 같기도 하고... 산모와 조산사의 입장 차이도 있겠고... 아무튼 세상의 모든 어머니는 위대합니다. 최소한 출산의 순간과 아이를 품에 안는 그 순간만큼은. . . . 도대체 내가 왜 이 일을 시작했을까? 아무래도 제정신이 아니다. 모델, 스튜어디스, 여객선 승무원... 매력적이고 돈도 많이 버는 다른 직업들이 머릿속을 지나간다. 이 일이 아니어도 내가 할 수 있는 일들. 새벽 두 시 반, 아직 비몽사몽인 채로 나는 끙끙대며 유니폼을 입는다. 바깥 날씨는 춥고 비가 내린다. 어둠 속에서 서둘러 자전거에 올라 분만 가방을 실은 채 인적이 끊긴 거리를 내달린다. 비를 맞으니 이제 좀 정신이 든다. 마음이 가라앉는다. 왜 하필이면 간호사가 되었을까? 소명의식 따위는 없었다. 환자를 구하겠다는 불타는 열정 같은 것도 없었다. 실연? 도피? 아니면 도전? 커프스와 주름칼라 그리고 허리까지 잘록해서 섹시해 보이는 유니폼? 작고 귀여운 모자? 모르겠다. 감청색 비옷을 입고 모자를 푹 눌러쓴 채 쉴 새 없이 자전거 패달을 밟으며 비를 뚫고 가느라 안간힘을 쓰고 있다. 참 섹시하기도 하겠다. 웨스트페리 대로를 벗어나 골목길로 들어선다. 날 기다리는 산모의 집이 한눈에 들어온다. 딱 한 집만 불이 켜져 있기 때문이다. 산모의 어머니와 할머니가 있다. 숙모님들, 자매들, 친한 친구들과 이웃 사람들도 보인다. 마치 나를 맞이하려고 기다리는 여성 사절단 같다. 든든한 여성 사절단 너머 어딘가에 이 모든 소란의 원인 제공자가 혼자 숨어 있을 것이다. 이런 상황에 처한 남자는 언제나 안쓰럽다. 자신이 정말 하찮은 존재처럼 느껴질 테니 말이다. 2층에 있는 뮤리엘에게 갔다. 이 풍만한 가슴을 지닌 스물다섯 살 여인은 곧 넷째 아이를 출산할 것이다. 낮에 먼저 뮤리엘을 면도해둔 건 정말 잘한 일이다. 면도와 함께 관장도 했다. 관장은 정말 하기 싫은 일이다. 누가 비눗물을 1리터나 쓰면서 다른 사람을 관장해주고 싶을까? 특히나 화장실도 없는 집에서라면 더더욱 고역이다. 뮤리엘을 살펴보니 이제 분만 2기에 들어가고 있다. 땀, 가벼운 헐떡거림, 젖 먹던 힘까지 내서 온몸에 힘을 주느라 기이하게 일그러진 얼굴. 그래도 뮤리엘은 기적처럼 아기를 낳을 것이다. 뮤리엘은 아무 말도 하지 않는다. 그저 내 손만 꽉 쥐고 힘없이 미소를 짓는다. 어느 정도 분만이 진행되었는지를 알려면 질 검사를 해야 한다. 내가 손을 박박 문질러 씻는데 뮤리엘이 다시 진통을 시작한다. 진통은 산모의 연약한 몸이 완전히 벌어질 때까지 점점 더 강해진다. 분만이 최고조에 달했을 때 자궁은 수축하면서 순간 흡사 지하철 문이 닫히는 정도의 강한 압력을 가한다. 산모가 진통하는 모습을 지켜보면 충분히 알 수 있다. 나는 장갑을 끼고 손에 윤활유를 바른다. 뮤리엘은 내가 무엇을 왜 하려는지 정확히 알고 있다. 소독한 시트를 뮤리엘의 엉덩이 밑에 깔고 손가락 두 개를 질 속으로 가만히 집어넣는다. 태아의 머리는 잘 내려왔고 얼굴은 앞을 향하고 있다. 자궁경부에 얇은 테가 남아 있지만 분명 양수는 아직 터지지 않았다. 나는 태아의 심장소리에 귀를 기울인다. 규칙적으로 130, 양호하다. 트레이를 준비하고 작업대로 쓸 서랍장도 미리 깨끗하게 치워두었다. 가위, 탯줄을 고정할 죔쇠, 탯줄을 묶을 끈, 태아용 청진기, 거즈와 면봉, 동맥 겸자를 늘어놓는다. 많은 게 필요한 건 아니다. 들고 다니기 쉬워야 하고 자전거에 실을 수 있어야 한다. 진통은 이제 삼 분마다 오고 있다. 뮤리엘은 얼마나 더 견딜 수 있을까? 어떤 여자가 이보다 더 큰 고통을 참을 수 있을까? 갑자기 양막이 터지고 양수가 침대에 쏟아져 나온다. 적절한 타이밍이다. 이보다 양수가 일찍 터졌으면 솔직히 좀 걱정스러웠을 것이다. 진통이 잠시 멈춘 틈을 타 뮤리엘의 어머니와 함께 재빨리 젖은 시트를 간다. 이때 산모는 일어서지도 못하는 상태니 우리가 뮤리엘의 몸을 굴려야 한다. 다음 진통이 오자 머리가 보인다. 이제 온 정신을 집중해야 할 때다. 산모는 동물적인 본능으로 힘을 주기 시작한다. 두 번 이상의 출산 경험을 한 임산부들은 종종 몇 초 만에 태아의 머리를 밖으로 밀어내기도 한다. 그러나 그게 좋은 것은 아니다. 능숙한 조산사라면 반드시 태아의 머리가 천천히 나오게 하려고 애쓴다. 진통이 지나간 뒤 다시 청진기를 갖다 댄다. 이번에는 140. 여전히 정상이지만 심장 박동이 세졌다. 뱃속의 아기도 세상에 나오기 위해 힘겹게 애를 쓴다. 다시 진통이 온다. "조금만 밀어요 뮤리엘. 너무 세게 힘주지 말고. 아기가 곧 나올 거예요." 산모는 진통 때문에 제정신이 아니다. 하지만 분만의 마지막 순간 산모는 말 못할 환희를 느낀다. 환희를 느끼는 순간, 고통은 아무것도 아니다. 다시 진통이 온다. 아기의 머리가 너무 빨리 나온다. 너무 빠르다. "힘주지 말아요 뮤리엘! 빠른 호흡만 해요. 들이쉬고, 내쉬고. 지금처럼 계속." 아기의 머리가 너무 갑자기 나오면 회음부가 찢어질 수 있다. 그런 일이 생기지 않도록 나는 아기의 뒤통수를 잡았다. 진통과 진통 사이에 아기의 머리가 천천히 나오는 게 정말 중요하다. 나는 아기의 뒤통수를 잡은 채 분만에 집중했다. 그 순간 뜨거운 열기와 강렬한 기운에 진땀이 흘렀다. 분만이 임박했다. 나는 오른 손바닥의 불룩한 부분을 항문 뒤로 가져걌다. 그리고 아기의 정수리 때문에 외음부가 보이지 않을 때까지 앞으로 힘주어 지속적으로 밀었다. "다음 진통 때 머리가 나올 거예요. 지금 한 번에 힘주지 말아요. 그냥 몸이 알아서 하게 두세요. 그냥 마음을 가라앉히고 빠른 호흡만 해요." 이제 빠른 속도로 다음 진통이 올 것이다. 나는 마음을 단단히 먹는다. 머리가 회음부 밖으로 조금씩 나온다. 마침내 머리가 완전히 다 나왔다. 우리 모두 안도의 한숨을 쉬었다. 뮤리엘도 기진맥진해 있다. "잘했어요, 뮤리엘. 정말 잘하고 있어요. 이제 다 끝나가요. 다음 진통 때는 딸인지 아들인지 알 수 있을 거예요." 울긋불긋한 아기 얼굴은 피와 점액으로 뒤덮여 있다. 심장박동을 체크하니 정상이다. 아기의 머리가 음부를 통해 제대로 나왔는지 살펴본다. 음부 사이로 어깨가 보이는 걸 보니 곧 나올 수 있겠다. 다시 진통이 온다. "지금이에요 뮤리엘, 힘을 주고 밀어내요. 세게!" 나는 밖으로 드러난 아기의 어깨를 앞으로 위로 미끄러지듯 살살 꺼냈다. 반대쪽 어깨와 팔이 뒤이어 나왔다. 그리고 아기 몸뚱이 전체가 스르르 빠져나왔다. 뮤리엘의 어머니가 큰 소리로 외쳤다. "귀여운 왕자님이네 그려! 주님 감사합니다. 어때요? 건강하지요?" 뮤리엘이 기쁨의 눈물을 흘린다. "아, 다행이에요! 얼굴 좀 보여주세요. 세상에, 정말 사랑스러워요." 나도 뮤리엘만큼이나 가슴이 벅차올랐다. 무사히 아이를 낳았다는 안도감은 이렇듯 강하다. 나는 아기와 산모를 연결한 탯줄을 두 군데 조이고 그 사이를 잘랐다. 아기의 발목을 잡고 거꾸로 들어올렸다. 끈적거리는 점액이 목을 막고 있지는 않은지 확인했다. 아기가 숨을 쉰다. 아기는 이제 독립적인 하나의 생명이다. 나는 수건으로 아기를 감싸 뮤리엘에게 건네주었다. 뮤리엘은 아기를 고이 안고 입 맞추었다. 그리고 다정하게 속삭인다. "우리 사랑스럽고 어여쁜 천사." 솔직하게 말해 갓 태어난 아기들은 아름다운 것과는 거리가 멀다. 피를 뒤집어쓰고 푸르뎅뎅하고 눈도 제대로 못 뜬 상태다. 하지만 아기를 낳은 엄마 눈에는 그렇게 보이지 않는다. 완벽한 아기다. 아직 내 임무는 끝나지 않았다. 태반이 나와야 한다. 태반이 자궁에서 완전히 떨어져서 찌꺼기 하나 남김없이 깨끗하기 나와야 한다. 그렇지 않으면 산모에게 심각한 일이 발생한다. 감염되거나 과도한 출혈로 치명적인 상황이 벌어질 수도 있다. 어떤 분만에서든 태반을 꺼내는 일이 가장 어렵다. 출산이라는 막중한 일을 성공적으로 끝마치고 나면, 자궁의 근육도 휴일처럼 놀고 싶은가 보다. 십 분에서 십오 분 정도 더 이상 수축하지 않는 경우가 많다. 그 사이 산모는 가만히 누워서 아기를 안고 어르느라 자신의 아랫도리에서 무슨 일이 벌어지는지 전혀 모른다. 하지만 조산사는 이때가 가장 걱정스러운 순간이다. 자궁 수축이 다시 시작되더라도 아주 약한 경우가 많기 때문이다. 태반이 깨끗이 떨어져 나오게 하려면 타이밍을 잘 보고 신중하게 판단해야 한다. 흔히 능력 있는 조산사가 되려면 7년 정도의 경력이 필요하다고 한다. 그런데 나는 이제 1년차다. 그리고 지금 여기에 조산사라곤 나밖에 없다. 한밤중인데다 산모와 가족은 완전히 나에게 의지하고 있다. 게다가 이 집에는 전화도 없다. 하느님! 제발, 실수하지 않게 해주세요! 기도가 절로 나왔다. 자궁 아래쪽이 부드럽고 넓게 느껴진다. 태반은 여전히 자궁 위쪽에 붙어 있는 게 분명하다. 질 밖으로 나와 있는 탯줄을 외음부 바로 아래쪽에 묶어놓는다. 그러면 탯줄이 언제 길어지는지 볼 수 있다. 태반이 떨어져 자궁 아래쪽으로 내려오면 탯줄이 길게 늘어진다. 종종 뉴스에서 택시 기사나 버스 차장이 아기를 받았다고 할 때가 있다. 하지만 태반까지 어떻게 했다는 이야기는 없다. 전문가가 아닌 사람도 응급 상황에서 아기를 받을 수는 있다. 하지만 분만 3기, 즉 태반 반출기에 어떤 조치를 취해야 하는지를 짐작이라도 하는 사람이 몇이나 있을까? 대다수 사람들은 탯줄을 잡아당겨 태반을 꺼내려들 것이다. 하지만 그랬다간 엄청난 후폭풍을 감당해야 한다. 자궁이 수축하면서 근육을 당기는지 자궁 아래쪽이 단단해지고 뮤리엘의 배가 살짝 올라왔다. 지금인 것 같기도 하고 아닌 것 같기도 하다. 수축된 자궁치고는 너무 말랑했다. 다시 기다려본다. 다시 자궁이 수축했다. 손으로 만져보니 자궁 아래쪽이 불룩하고 단단해졌다. 동시에 탯줄을 묶어놓은 죔쇠가 조금 움직인다. 죔쇠를 당겨보았다. 탯줄이 10센티 정도 쉽게 나온다. 마침내 태반이 분리된 것이다. 뮤리엘에게 아기를 어머니에게 맡기라고 했다. 뮤리엘은 내가 무얼 하려는지 알고 있다. 나는 손으로 뮤리엘의 아랫배를 살살 문질러 태반이 둥그렇고 단단하게 뭉쳐지게 했다. 그런 다음 배를 꽉 움켜쥐고 골반까지 아래로 쭉 밀었다. 그러자 태반이 외음부 바깥으로 나오기 시작했다. 양막이 미끄러지듯 빠져나오고 생피와 엉긴 피가 뒤이어 왈칵 빠져나왔다. 안도감에 맥이 풀린다. 다 끝났다. 뮤리엘 옆에 앉아 십 분 넘게 아랫배를 마사지해 주었다. 아직 아랫배가 단단하고 둥굴다. 자궁에 남아 있는 핏덩이가 계속 더 빠져나올 것이다. 요즘에는 분만 3기에 자궁 수축제를 많이 쓴다. 자궁 수축제를 쓰면 곧바로 자궁이 수축한다. 분만 뒤 3~5분이면 태반이 떨어져 나온다. 의학기술의 발달은 참으로 빠르다! 하지만 1950년대 우리가 일할 때는 그런 분만 보조제가 전혀 없었다. 이제 치우는 일만 남았다. 호킨 부인이 딸을 씻기고 옷을 갈아입혔다. 그동안 나는 아기를 씻기고 옷을 입혔다. 옷이 너무 커서 우스꽝스러워 보인다. 뮤리엘이 행복에 겨워한다. 얼굴 가득 평화와 온기가 흐른다. 뮤리엘은 지쳐 보이긴 해도 스트레스나 부담감의 흔적은 없어 보였다. 여자의 몸속에는 망각을 도와주는 시스템이 있는 게 분명하다. 아기를 낳고 나면 바로 뇌에서 호르몬이나 화학물질 같은 것이 분비될 것이다. 그렇지 않고서야 이 세상 어떤 여자가 다시 아기를 가지려고 할까? 모든 것을 정리하고 나서야 아기 아빠를 부른다. 요즘은 남편들이 분만 내내 아내 곁에서 아기가 태어나는 장면을 지켜본다. 하지만 그건 정말 최근의 일이다. 1950년대에는 분만할 때 남편이 함께 있었다는 이야기를 들으면 누구라도 큰 충격을 받았을 것이다. 출산은 여성의 일이었다. 아기 아빠인 짐은 몸집이 작았다. 서른도 채 안 되었을 텐데 겉모습은 거의 마흔쯤으로 보였다. 부두에서 주당 60시간씩 일해서 저런 모습일 거다. 뮤리엘의 깨끗하고 맑은 피부 때문에 짐은 더 늙고 초췌하고 야위어 보인다. 멋쩍은 얼굴로 어찌할 바를 몰라 하며 조용히 들어온 짐은 아기를 보자 살짝 콧노래를 흥얼거렸다. 분명 적당한 말을 찾느라 고민하는 것이다. 목청을 가다듬더니 "세상에! 정말 잘생겼네." 하고는 방에서 나갔다. 이스트엔드의 남자들은 우리 조산사에게 예의를 갖추고 늘 존중해주었다. 하지만 그건 우정이나 친숙함의 표시가 아니었다. 그 당시의 남자들은 남성이 해야 할 일과 여성이 해야 할 일을 엄격히 구분했다. 심지어 제인 오스틴조차도 여자라는 이유로 남자끼리 주고받는 대화에 전혀 낄 수도 알 수도 없었다. 그래서 작품에 남자끼리의 대화는 전혀 쓰지 않았다고 한다. 나 역시 제인 오스틴처럼 남자들에 관해서는 자세히 기록할 수가 없다. 그저 겉으로 보이는 것만 기록할 뿐이다. 나는 떠날 채비를 마쳤다. 길고 긴 하루였다. 그래도 보람차고 흐뭇한 느낌에 발걸음은 가볍고 마음은 홀가분했다. 뮤리엘과 아기가 함께 잠든 모습을 보고 조용히 방에서 빠져나왔다. 아래층에 모인 사람들이 차를 권했다. 하지만 나는 정중히 거절하고 혹시 무슨 문제가 생기면 곧바로 알려달라고 이야기했다. 이제 아침 햇살 속으로 발을 내딛는다. 아직 거리는 아무도 없다. 텅 빈 부두를 자전거를 타고 달려 지나갔다. 문득 인생이 낯설게 느껴졌다. 어린 시절은 전쟁 때문에 엉망 진창이었다. 열여섯 살에 연애를 했다. 삼 년 뒤에는 그 뜨거운 연애에서 도망쳐야 한다는 걸 알았다. 순전히 한 남자로부터 도망치기 위해 간호학을 선택했다. 지금 나는 후회하는 걸까? 날카로운 소리에 상념에서 깨어났다. 위협적으로 달리는 화물트럭을 피하려고 자전거를 인도로 바짝 붙였다. 커다란 덩치에 강철같이 단단한 근육질 남자가 모자를 벗더니 내게 소리쳤다. "안녕하슈, 간호사 선생!" "안녕하세요, 멋진 날이에요!" 나도 큰 소리로 인사했다. 젊음, 아침 공기, 부둣가의 자극적인 흥분, 무엇보다 산모를 도와 예쁜 아기를 분만시켰다는 충만함! 들뜬 마음으로 쉬지 않고 자전거 패달을 밟았다. 절대로 후회하지 않는다. 나는 이 일을 세상 무엇과도 바꾸지 않을 것이다.
