* 본 이야기는 저의 가족사이며 허구는 없습니다. 공부하다가 잠깐 쉬는데 옆동네에서 의료이야기가 나와서 한번 써봐야지 했던걸 써봅니다. 예전에 주변에 의료사고가 자주 있다 라고 써놨더니 그런게 어딨냐, 의료사고가 그렇게 빈번하지 않다는 의사분들이 많았어서 (여기였는지 다른동네였는지 기억은 안납니다만) 이걸 써야지 했었는데.. 여튼. 짧게 풀어봅니다. 1.할머니 할머니께서 일흔이 좀 지나시고 쓰러지신적이 있었습니다. 당시 사시던곳은 군보다는 크고 시보다는 작지만 시내에 종합병원이 두어군데 있었어요. 할머니가 쓰러지신지 시간이 좀 지나서 발견이 되셨고, 근처 종합병원에서 1차 처치를 받으셨죠. 근데 처음에 진단이 잘못내려져서, 사태가 좀 안좋게 흘러갔습니다. 당시 제가 중1이니 정확하게 기억이나 병명을 알 수 있는 상황은 아니었죠. 1차 처치를 빠르게 받으셨어야했는데, 처음 종합병원에서 시간을 많이 끌었습니다. 여기서 수술을 하느냐, 더 큰 병원으로 옮기느냐. 더 큰 병원으로 옮겨야 할 일이냐. 이런걸 두고 정확한 오더가 나오질 않았어요. 할머니께선 뇌출혈이셨는데, 그게 정확하게 진단이 되지 않았나봐요. 그래서 뒤늦게 구급차로 다른 병원으로 이송하려고 했는데, 병원에서 나오는데 할머니 상태가 이상하셨답니다. 의식을 잃은 상태인데 호흡을 못하시는거죠. 알고보니까 누운 상태로 구토를 하셨는데 병원 배드에서 그걸 캐치를 못한거에요. 그래서 뒤늦게 기도를 확보하고 이송을 했고, 결국 도 전체에서 가장 큰 대학병원에서 수술을 받았죠. 몇시간 단위가 아니고 무려 하루 반나절이 걸렸던 일입니다. 다행히 생명은 건지셨지만, 가족들 입장에선 황당한 일이었죠. 종합병원 두군데를 다 돌고서야 대학병원까지 이송했는데 그 시간동안 계속 나빠졌으니. 그 뒤로 십년쯤 더 사시다 가셨지만 그 일 이후로 건강하고 씩씩하고 기운넘치셨던 할머니가 걷는데도 보조기를 이용해야 되고 치매에 가까운 증상을 겪으셨던것도 사실이죠. 2.아버지 젊으실적에 운동을 워낙 좋아하시던터라, 이 종목 저 종목 가리지 않고 뛰셨습니다. 하루는 배구를 하다가 상대와 네트에서 접촉을 하며 떨어질때 무릎을 다치셨어요. 십자인대의 파열.. 이었는데. 당시 처음 진단내린 의사가 수술적 치료를 권했습니다. 그런데 자기는 수술을 할 수가 없다며 다른 병원으로 보냈어요. 그랬더니 거기서는 수술해도 별로 좋아질 것이 없다며 비수술적 치료를 하자고 했죠. 의료서비스는 워낙 전문적인 판단을 요하고, 그래서 우리가족은 누구의 말을 듣는게 더 합리적인 판단일지 정보를 알 수가 없었습니다. 결국 칼을 대는것보다 보존적 치료를 하는게 낫지 않겠냐는 쪽으로 얘기가 흘러갔고, 반년이 넘게 입원치료를 했죠. 지금도 아버지는 달리기같은 운동은 못하십니다. 종종 그런생각을 하죠. 십자인대가 파열되도 수술하고 재활하면 잘 뛰어다니는데, 왜 그때 그 의사들은 의견이 갈렸던걸까. 3. 작은아버지 얼마전 작은아버지께서 2층에서 작업을 하다 떨어지셨습니다. 문제는 이게 크레인의 와이어가 끊어지면서 추락한거라 크게 다치셨는데, 한쪽 팔과 어깨로 떨어지며 그쪽을 크게 다치셨다고 하더라고요. 처음 병원에 갔더니 팔뼈가 다 박살나고 신경이 조각났다나, 여튼 신경과 뼈, 근육이 너무 많이 상해서 종합병원으로 이송을 했고, 종합병원에서는 수술을 했습니다. 신경과 뼈를 복원하는 장시간의 수술이었죠. 10주~12주정도면 완치를 바라볼 수 있고 8주정도 입원해있어야 한다고 하더군요. 이후에 재활도 해야하고. 그런데 한달이 지나도 붓기가 전혀 가라앉지 않고 통증도 심하고 손가락도 잘 안움직이시더랍니다. 원래는 8주정도면 깁스를 반깁스로 바꾸고 어느정도 생활이 가능해서 퇴원을 바라볼 시기라고 안내를 받았는데 이상했죠. 다시 검사를 받아도 별 문제는 없다는 얘기만 반복하고. 10주를 채우고도 진통제에 의존해야 해서 결국 병원을 바꿔서 검사를 했습니다. 왠걸, 신경은 10%가 채 안남고 다 죽어있고, 근육과 뼈도 제대로 회복이 안되는 상태였어요. 재수술이 필요하단 소견을 받았죠. 그래서 서울 연대 세브란스까지 다시 왔습니다. 어차피 재수술이라면 조금이라도 더 제대로된 검사를 받고싶어서요. 진단은 비슷했고 다음달에 수술이 다시 잡혔네요. 이 외에도 여러가지 수술경험들이 가족들에게 많습니다. 잘 된것도 있고, 잘 안된것도 있죠. 그렇다고 우리가족이 의료소송을 하거나 한 적은 없고, 그정도의 의료과실이냐 하면 감정적인 것 이상으로 '최선을 다했다'는 말에대한 의심을 어떻게 증명하느냐의 문제도 있고. 사실 의사'선생님'이라 믿고 따랐으니 감히 고소할생각도 못하실 분들이었고. 사촌동생은 팔꿈치를 다쳤는데 재수술을 했고, 손목을 다쳤을땐 네번을 했어요. 다 처음 수술할때는 문제 없을거라던 수술들이었고, 수술을 해준 병원들은 마지막까지도 환자의 불편함에 검사상 문제가 없다고 했었죠. 수술자국이 서너개가 남아야 치료가 되더군요. 물론 가족들이 의료서비스를 이용하는 입장에서 훨씬 더 정확하고 올바른 처치와 서비스를 받은 경우가 많았다는걸 잘 압니다. 그런데 이런게 사실 '한 건'이 잘못되면 크게 와닿는 거잖아요. 이성적으로야 많이 째고 많이 실수하고 하면서 사람이 성장하는거고, 의사도 실수를 할 수 있고, 고도의 판단이다보니 잘못된 결과로 이뤄질 확률또한 있다는거야 알지요. 근데 사람의 행동은 어느정도는 불확실하고 어느정도는 불성실하잖아요. 완전히 확실하고 성실한 사람의 비율만큼이나 완전히 불성실하고 불확실한 사람이 적은 것처럼. 저는 위 사례들을 거친 의사분들이 얼마나 최선을 다한건지 어떤 전문성이 있었는지는 잘 모르겠습니다. 결과적으로 세분 다 치료를 받았으니 의료서비스 자체의 목적은 달성한 느낌인데, 중간과정이 참 지난했던것 같아요. 그러니 이런 경험이 있는 집의 어른들이 한의학에 빠지고 이러는게 이해가 안되는건 아닙니다. 비이성적인 판단인데, 감정적으로는 그럴수도 있겠다 싶은거요. 여하튼, 사람이라는게 참 간사해서 저런 경험들을 겪고나서 의료보험제도 개혁, 의료수가제개혁, 의사 근무환경개선, 의료산업체질개선 이런 소릴 들으면 반대부터 하신단 말이에요. 주장 자체가 말이 안되서라기보단, 감정적인 이유겠죠. 저는 의료산업의 문제가 전반적으로 많은게 엮여서 무엇부터 해결해야하는지가 어려운 문제라고 생각해요. 이걸 단순히 시장주의와 자유주의에 편입시켜서 바라볼 것이냐, 공공서비스나 사회서비스, 윤리적 문제를 끝까지 끌고갈것이냐 같은 방향성의 문제부터 의사 개인의 삶에 있어서 어디까지를 권리로서 존중할 수 있고, 환자 개인의 생명권이나 의료서비스에대한 권리역시 어디까지 선을 그을것인가. 그리고 의사 개인이 처하는 척박한 노동현실과 의사가 되기위한 개인적 수련과정들이 과연 사회적 책임만큼 사회와 공유되어 있는가같은 문제까지. 무엇을 우선으로 두느냐도 문제고, 무엇을 어떻게 대중에게 설득할 것인가도 문제겠죠. 사실 그렇잖아요. 모든 노동자의 권리증진은 모든 사회적 비용과 사용자의 부담을 증가시킵니다. 권리의 진보는 결국 무언가로부터의 비용을 더 만들어내거나 빼앗는 과정을 설득하는 과정이잖아요. 아니면 힘으로 꺾어내거나. 의료수가제 관련 싸움과 의료진을 상대로하는 폭력문제들, 의료인의 과다노동문제, 진료거부권 같은 쟁점들과 동시에 외상센터와 기피과목들 문제, 구급의료와 응급의료 수요공급문제, 의료사고와 공정한 법적인 체계의 문제, 병원과 환자의 정보비대칭성과 의료분쟁문제 같은것들은 동전의 양면같다는 생각을 해요. 그래서 전 이게 굳이 헬조선이라서, 환자가 진상이라 혹은 의사가 악독해서, 의료계의 직업윤리나 사회적 책임 이런것과는 좀 다른 결의 사회적 문제라는 생각을 합니다. 다만 의료산업에 종사하는 사람과, 의료산업을 이용하는 사람의 온도차를 줄이는건 의료산업을 이용하는 사람이 스스로 깨우쳐 계몽하는 일 따윈 없으며.. 여전히 큰 온도차가 있다는 부분을 이야기하고 싶었어요. 그리고 의사가 그나마 '선생님'소릴 최근까지 듣는직업이라 그렇지, 사실 이미 거의 대부분의 임금노동자들에게는 여론, 사회적 시선, 대중의 인식을 노동자 '당사자'들이 투쟁으로, 죽음으로, 울음으로, 고통으로, 비명으로, 제도로, 정치로, 언론으로 발악을 하거나 순응하는게 일상인 사회에서 과연 의료산업은 어떤 방향으로 흐를지 하는 생각도 드네요. 개인 역량이 국제적 경쟁력이 되는, 자본의 이동만큼이나 타 노동시장으로의 이전이 유리한 직종이니만큼 어쩌면 여타 이동이 어려웠던 노동자 집단에 비해 개별적 대책이 훨씬 크게 두드러질지도 모른다는 생각도 들고, 그렇습니다. 앞으로 살면서 큰 병 앓아서 병원다닐일이 없으면 좋겠지만, 그런일이 생기게되면 기댈곳은 의사의 선의와 운밖에 없다는것도 좀 절망적이네요. 일단 주변 분들의 안좋은 경과에 대해서 (오래된 얘기긴 하지만) 위로의 말씀을 전합니다.
근데 저는 이 글에서도 너무나 당연할 수 있는 시각차가 느껴집니다. 1-3번은 써있는 정황으로 보면, 어떠한 사고/과실이 없이도 진행될 수 있는 시나리오로 보입니다. 그러니까 본문의 정황으로 알 수 있는건 개인에게 매우 기분이 좋지 못할 인생의 비극적 사건일 수 있겠구나 라는 점이지, 과연 저것이 특정한 주체의 책임에 의해서만 일어날 수 있는 일들인가 좀 의문이 듭니다. 그리고 실제로 서비스/접객 관련쪽에서 상식적으로 이야기하는 부분이지... 더 보기

그리고 음... 이건 전적으로 제 개인적인 견해이고 오히려 의사 집단 내부에서는 소수의 의견입니다만, 저는 환자가 제게 고마워하는 것도 원치않고 환자 혹은 잠재적 환자인 시민들이 의사를 특정한 성직 같은 것으로 여기길 원치도 않으며, 진료 모델에 있어서도 방관자적 컨설턴트를 긍정적으로 바라보지 고전적인 교사-학생 모델을 지향점으로 삼는 일은 없어야 한다고 봅니다. 그렇기 때문에, 다른 직종이 그러한 것처럼, 의사 역시 능력껏 알아서 돈을 벌 수 있는 구조를 만들어 주거나, 아니면 걍 보험공단과 심평원이 시키는대로 진료볼테니까 대신 그러다 죽어도 정부에서 책임지는 걸로 만들어주거나 했으면 좋겠습니다. 책임은 내가 지고 결정은 남이 하는건 헬조센에서도 '일반적'이진 않아 보입니다. 직장에서는 일반적이지만, 적어도 직종이라는 점에서는요.
