- 그렇죠. 보조적인 거고, 해열제로 우리 뇌의 체온을 조절하는 중추의 이미 셋팅이 된 체온값을 떨어뜨리지 않고서는 미온수 마사지만 하면 원래 셋팅된 체온을 유지하려고 계속 몸을 떨게 됩니다. 아이가 더 힘들어지죠. 해열제를 먼저 쓰고 미온수 마사지를 한다, 그거를 기억해 주셨으면 좋겠고요.
미온수 마사지를 보면 사실 욕탕에 미지근한 물이나 따뜻한 물을 받아 놓고 아이를 담그죠. 그렇게 하는 경우가 있는데 그렇게 해도 일부 체온을 떨어뜨리는 효과는 있겠지만, 가장 좋은 건 공기 중에서 물이 증발하면서 열을 같이 뺏어가는 효과를 기대해야하기 때문에, 아이를 2cm나 2.5cm 정도로 물을 얕게 받아 놓고 아이를 거기 앉히고 수건이나 거즈로 아이의 넓은 등이나 가슴을 닦아주면서, 물이 증발하면서 체온을 뺏어가도록 그 효과를 노리면서 해주는 게 가장 중요한 방법이 되겠습니다.
Q. 그런데 아까 말씀하신 대로 애가 떨고 오한이 있고 이러면, 그때도 계속합니까?
- 아이가 힘들어 한다. 이거 좋아하는 애들이 별로 없거든요. 이거 많이 힘들어하고, 특히 떤다, 그때는 이 미온수 마사지는 전혀 효과가 없는 겁니다. 아이만 괴롭히는 거죠. 그러면 미온수 마사지는 중단하시고, 오한이 있을 땐 얇은 옷으로 아이를 덮어주시면서 아이를 편하게 해주시는 게 제일 좋은 방법입니다.
- 결국 핵심은 아이들의 발열을 잡기 위해서 사용할 수 있는 방법 중에 중요한 건 해열제가 우선인 거지 미온수 마사지가 우선은 아니라는 걸 꼭 기억하셔야 겠네요.
Q. 소아 발열에 대해서 계속 얘기하고 있는데 그 얘기를 조금 더 깊이 들어가서 사실 응급실에서 일하다 보면, 열성 경련, 즉 아이들이 열이 나면서 발작, 경련을 해서 응급실을 찾는 경우들이 적지 않거든요. 이렇게 열성 경련이 있을 때 빨리 아이를 데리고 응급실에 와야 되겠지만, 집에서 할 수 있는 일이 있을까도 궁금하고요. 특히 지금까지 얘기한 열을 조절하기 위한 해열제 투여나 아니면 미온수 마사지 같은 걸 하면서 또는 하고 오는 게 좋은지 아니면 그냥 빨리 응급실로 오시는 게 나은 건지 등등 열성 경련과 관련 되서도 말씀 좀 해주시면 좋겠습니다.
- 부모님들이 가장 놀라는 질병 중 하나죠. 경련인데요, 특히 열성 경련은 열 명에 한 명까지도 보고가 될 정도로 굉장히 흔해서 부모님들한테 저희가 교육을 꽤 많이 하고 있습니다. 잘못된 응급처치들이 몇 가지가 있는데 첫 번째가 경련을 하는 아이한테 뭘 먹이시는 거죠. ‘놀랐다! 기웅환’, 또 ‘열난다! 해열제’. 경련을 하는 동안에는 기도를 보호하는 반사가 다 사라집니다. 그래서 뭔가가 입으로 들어가면 기도로 넘어갈 위험이 굉장히 높은 거죠. 그래서 절대 경련을 하는 도중에는 아무 것도 입으로는 먹이지 마셔야 되고요. 이게 가장 중요한 응급처치의 첫 번째가 될 겁니다.
두 번째로는 아이의 팔다리를 주무르거나 경련을 하는 팔다리를 꽉 잡고 계시는 경우가 있죠. 그거는 경련을 멈추는 데에는 전혀 도움이 되지 않습니다. 굳이 그렇게 하실 필요는 없고, 집에서 해줄 수 있는 응급처치는 기도를 보호하는 게 될 거고요. 경련을 하다가 토하는 애들이 있고 또는 뭐를 먹다가 경련을 할 수도 있고요. 아이의 얼굴을 한 쪽으로, 오른 쪽이나 왼 쪽으로 돌리시고 입 안에 음식물이 의심이 되면 손을 넣어서 빼주시고 그래서 기도가 막히지 않도록 해주는 게 가장 중요한 응급처치가 되겠습니다. 또 경련 중에 다칠 수가 있어요. 주위에 부딪힐 만한 거 또는 아이가 건드려서 떨어질 만한 거 그런 것들이 있을 수 있으니까 그런 장소를 피해서 아이를 옮겨 주시고, 다칠 만한 물건들을 치워 주시고, 이렇게 두 가지가 중요한 응급처치가 되겠네요.
Q. 교수님 말씀 중에 중요한 건, 우리나라 사람들이 어디가 아프거나 힘들면 뭘 자꾸 먹여서 해결하려고 하는데 안 먹이는 게 좋을 때가 있는데, 특히 아이들한테 억지로 먹이다가 기도로 넘어가게 되면 폐렴 생기고 굉장히 힘들어 지니까 절대 하시지 말아야 되겠고, 아까 말씀 중에 특히 입 속에 음식 같은 게 있어서 제거하실 때도 잘못하면 밀어 넣는 경우가 있거든요. 손가락을 넣다가 밀어 넣는 경우가 있으니 절대 그러지 않도록 하셔야 될 것 같습니다. 응급처치를 설명하다 보면 해야 할 것보다는 하지 말아야 될 게 많은 것 같아요.
- 굉장히 중요한데요. 뭘 자꾸 하려고 그러는 것보다는 안 해야 될 걸 지키는 것도 건강을 위해서 필요할 것 같습니다.
- 열성경련에 대해 하나 더 말씀드리면 잘 관찰하는 게 중요합니다. 열성경련 거의 대부분이 5분 안에 멈추거든요. 그런데 저희가 ‘열성경련 아이들 중에 검사를 해야 되겠다, 이게 혹시 간질이라고 하는 어떤 병적인 상황과 관련이 있을 수도 있겠다’ 판단을 해야 될 경우가 있는데 그러려면 경련할 때의 양상이 굉장히 중요하거든요. 그래서 부모님들이 아이가 경련할 때 팔다리 떠는 거, 양 쪽이 똑같이 떠는지, 눈동자가 돌아가는지, 침을 흘리는지, 입 주위가 파란지, 잘 보시고, 시간도 가능하면 보시고 병원에 와서 잘 얘기를 해주시면 아이 진찰하고 진단하는 데 굉장히 도움이 되죠.

- 그렇죠. 보조적인 거고, 해열제로 우리 뇌의 체온을 조절하는 중추의 이미 셋팅이 된 체온값을 떨어뜨리지 않고서는 미온수 마사지만 하면 원래 셋팅된 체온을 유지하려고 계속 몸을 떨게 됩니다. 아이가 더 힘들어지죠. 해열제를 먼저 쓰고 미온수 마사지를 한다, 그거를 기억해 주셨으면 좋겠고요.
미온수 마사지를 보면 사실 욕탕에 미지근한 물이나 따뜻한 물을 받아 놓고 아이를 담그죠. 그렇게 하는 경우가 있는데 그렇게 해도 일부 체온을 떨어뜨리는 효과는 있겠지만, 가장 좋은 건 공기 중에서 물이 증발하면서 열을 같이 뺏어가는 효과를 기대해야하기 때문에, 아이를 2cm나 2.5cm 정도로 물을 얕게 받아 놓고 아이를 거기 앉히고 수건이나 거즈로 아이의 넓은 등이나 가슴을 닦아주면서, 물이 증발하면서 체온을 뺏어가도록 그 효과를 노리면서 해주는 게 가장 중요한 방법이 되겠습니다.
Q. 그런데 아까 말씀하신 대로 애가 떨고 오한이 있고 이러면, 그때도 계속합니까?
- 아이가 힘들어 한다. 이거 좋아하는 애들이 별로 없거든요. 이거 많이 힘들어하고, 특히 떤다, 그때는 이 미온수 마사지는 전혀 효과가 없는 겁니다. 아이만 괴롭히는 거죠. 그러면 미온수 마사지는 중단하시고, 오한이 있을 땐 얇은 옷으로 아이를 덮어주시면서 아이를 편하게 해주시는 게 제일 좋은 방법입니다.
- 결국 핵심은 아이들의 발열을 잡기 위해서 사용할 수 있는 방법 중에 중요한 건 해열제가 우선인 거지 미온수 마사지가 우선은 아니라는 걸 꼭 기억하셔야 겠네요.
Q. 소아 발열에 대해서 계속 얘기하고 있는데 그 얘기를 조금 더 깊이 들어가서 사실 응급실에서 일하다 보면, 열성 경련, 즉 아이들이 열이 나면서 발작, 경련을 해서 응급실을 찾는 경우들이 적지 않거든요. 이렇게 열성 경련이 있을 때 빨리 아이를 데리고 응급실에 와야 되겠지만, 집에서 할 수 있는 일이 있을까도 궁금하고요. 특히 지금까지 얘기한 열을 조절하기 위한 해열제 투여나 아니면 미온수 마사지 같은 걸 하면서 또는 하고 오는 게 좋은지 아니면 그냥 빨리 응급실로 오시는 게 나은 건지 등등 열성 경련과 관련 되서도 말씀 좀 해주시면 좋겠습니다.
- 부모님들이 가장 놀라는 질병 중 하나죠. 경련인데요, 특히 열성 경련은 열 명에 한 명까지도 보고가 될 정도로 굉장히 흔해서 부모님들한테 저희가 교육을 꽤 많이 하고 있습니다. 잘못된 응급처치들이 몇 가지가 있는데 첫 번째가 경련을 하는 아이한테 뭘 먹이시는 거죠. ‘놀랐다! 기웅환’, 또 ‘열난다! 해열제’. 경련을 하는 동안에는 기도를 보호하는 반사가 다 사라집니다. 그래서 뭔가가 입으로 들어가면 기도로 넘어갈 위험이 굉장히 높은 거죠. 그래서 절대 경련을 하는 도중에는 아무 것도 입으로는 먹이지 마셔야 되고요. 이게 가장 중요한 응급처치의 첫 번째가 될 겁니다.
두 번째로는 아이의 팔다리를 주무르거나 경련을 하는 팔다리를 꽉 잡고 계시는 경우가 있죠. 그거는 경련을 멈추는 데에는 전혀 도움이 되지 않습니다. 굳이 그렇게 하실 필요는 없고, 집에서 해줄 수 있는 응급처치는 기도를 보호하는 게 될 거고요. 경련을 하다가 토하는 애들이 있고 또는 뭐를 먹다가 경련을 할 수도 있고요. 아이의 얼굴을 한 쪽으로, 오른 쪽이나 왼 쪽으로 돌리시고 입 안에 음식물이 의심이 되면 손을 넣어서 빼주시고 그래서 기도가 막히지 않도록 해주는 게 가장 중요한 응급처치가 되겠습니다. 또 경련 중에 다칠 수가 있어요. 주위에 부딪힐 만한 거 또는 아이가 건드려서 떨어질 만한 거 그런 것들이 있을 수 있으니까 그런 장소를 피해서 아이를 옮겨 주시고, 다칠 만한 물건들을 치워 주시고, 이렇게 두 가지가 중요한 응급처치가 되겠네요.
Q. 교수님 말씀 중에 중요한 건, 우리나라 사람들이 어디가 아프거나 힘들면 뭘 자꾸 먹여서 해결하려고 하는데 안 먹이는 게 좋을 때가 있는데, 특히 아이들한테 억지로 먹이다가 기도로 넘어가게 되면 폐렴 생기고 굉장히 힘들어 지니까 절대 하시지 말아야 되겠고, 아까 말씀 중에 특히 입 속에 음식 같은 게 있어서 제거하실 때도 잘못하면 밀어 넣는 경우가 있거든요. 손가락을 넣다가 밀어 넣는 경우가 있으니 절대 그러지 않도록 하셔야 될 것 같습니다. 응급처치를 설명하다 보면 해야 할 것보다는 하지 말아야 될 게 많은 것 같아요.
- 굉장히 중요한데요. 뭘 자꾸 하려고 그러는 것보다는 안 해야 될 걸 지키는 것도 건강을 위해서 필요할 것 같습니다.
- 열성경련에 대해 하나 더 말씀드리면 잘 관찰하는 게 중요합니다. 열성경련 거의 대부분이 5분 안에 멈추거든요. 그런데 저희가 ‘열성경련 아이들 중에 검사를 해야 되겠다, 이게 혹시 간질이라고 하는 어떤 병적인 상황과 관련이 있을 수도 있겠다’ 판단을 해야 될 경우가 있는데 그러려면 경련할 때의 양상이 굉장히 중요하거든요. 그래서 부모님들이 아이가 경련할 때 팔다리 떠는 거, 양 쪽이 똑같이 떠는지, 눈동자가 돌아가는지, 침을 흘리는지, 입 주위가 파란지, 잘 보시고, 시간도 가능하면 보시고 병원에 와서 잘 얘기를 해주시면 아이 진찰하고 진단하는 데 굉장히 도움이 되죠.
