경제학에서 삼원 불가능성의 정리(Impossible Trinity)라고 하면 보통 개방경제가 동시에 달성할 수 없는 세 가지를 의미합니다. 환율의 안정, 통화정책의 독립성, 자본이동의 자유화 중 어느 한 가지는 포기해야 한다는 것이죠. 이와 비슷하게 세 가지 중 두 가지를 달성하는 것은 가능하나, 셋 모두를 달성할 수는 없는 상황을 트릴레마(trilemma)라고 하는데, 비슷한 사례가 <보건의료의 철의 삼각형(Iron triangle of Healthcare)>입니다.  의료 서비스는 일반적으로 서비스의 질(quality), 비용(cost), 접근성(accessibility)을 동시에 달성하지 못한다고 알려져 있습니다. 이는 서비스를 제공할 수 있는 주체가 제한되어 있기 때문에 생기는 문제입니다. 만약에 보건 서비스의 질이 좋으면서 가격이 낮다면 우리는 조금만 아파도 병원에 갈 것입니다. 가장 쉽게 떠올릴 수 있는 예시는 군 병원입니다. 군 병원에서는 MRI와 CT가 무료다 보니 온갖 꾀병 환자들이 득실득실하고, 진짜 아픈 병사가 MRI를 찍고 싶어도 두 달은 외진을 들락날락 거리며 대기해야 합니다. 따라서 내가 원할 때 서비스를 받을 수 있는 정도(접근성)은 떨어지겠죠. 반대로, 접근성과 품질을 택한 미국 모델의 경우 고품질의 의료 서비스를 언제든 원할 때 받을 수 있지만 높은 사보험료를 감수해야만 가능합니다. 또한 유럽처럼 가격과 접근성을 택하고 수요를 감당할 만한 의사를 양산해서 배치하는 경우 서비스의 질적 하락을 피할 수 없다는 문제가 생깁니다. 그래도 이 글을 읽는 대다수의 독자분들은 한국의 의료 서비스가 품질, 가격, 접근성 중 어느 하나가 특별히 부족하다는 느낌을 평소에 받지 않을 것입니다. 한국은 사실 전 세계에서도 손꼽히게 고품질의 의료 서비스를 효과적으로 제공하고 있는 나라이기 때문입니다. 하지만 당연히, 여기에는 경제의 일반적인 법칙을 거스르는 비밀이 있지요. 한국이 보건의료의 철의 삼각형을 무너트릴 수 있는 기저에는 당연지정제/수가제와 징병제가 자리잡고 있습니다. 당연지정제와 수가제는 간단히 설명하면, 모든 병원과 모든 국민이 국민건강보험에 가입해야 하고, 나라에서 필수적이라고 지정한 의료 서비스의 가격은 심평원에서 정한 가격(수가)에 따라야 한다는 것입니다. 한편 현재 이 수가는 실제 가치에 비해 지나치게 낮게 설정되어 있고, 인건비를 지급하기에도 부족합니다. 경제학적으로 보면 가격통제이고 당연히 누군가는 손해를 보고 있습니다. 그것은 병원이지요. 대형병원일수록 수가의 영향을 받는 과의 비중이 높고, 현재 대학병원은 모든 병상을 항상 꽉꽉 채워도 대학병원에 고용된 의사, 간호사, 행정직원들의 인건비를 지급하기도 불가능합니다. 하지만 수가 현실화는 곧 건강보험료의 급등을 의미하고, 정부는 건강보험료 급등에 뒤이은 지지율 하락을 감당할 자신이 없기 때문에, 지속적으로 이 문제를 후대 정권에게 떠넘김과 동시에, 병원에게는 수입을 현실화할 우회로를 용인하고 있습니다. 대학병원에서 운영하는 장례식장이나 비싼 편의시설은 그런 모습 중 하나이고, 한 의사에게 지나치게 많은 환자가 배당된 5분 진료도 그런 모습 중 하나입니다. 의협 등으로 대표되는 의사단체들은 수술을 하면 적자가 발생한다는 모순을 해결하기 위해 수가 현실화나 영리병원 등을 주장하고 있지만 어쨌든 의사 니들은 돈 많이 벌면서 먹고 살 만하지 않느냐라는 논리 앞에서 여론전에서 패배를 거듭 중이죠. 문재인 정권의 공약이었고, 현재 실시중인 문재인 케어는 기존 비급여를 모두 급여의 영역으로 끌어안으면서 이 문제를 해결하기는커녕 더 악화시키는 방향으로 진행중에 있지만, 어쨌든, 그럭저럭 현재 건강보험 체제는 굴러가고 있습니다. 언제까지 지속될 지는 모르겠지만요. 한편 징병제 역시 이런 수요가 제한된 고급 서비스를 염가에 굴리는 데 톡톡히 기여하고 있습니다. 여러분들은 시골 의사와 도시 의사 중 누가 월급을 더 많이 받아야 한다고 생각하시나요? 저는 시골 의사라고 생각합니다. 왜냐하면 의사처럼 넉넉한 수입을 받는 사람들은 굳이 고생하면서 시골에 남아 있으려고 하지 않고, 모두가 도시에 사는 걸 선호하는 상황에서 의료수요가 많고 의료공급이 적은 시골에 가려면 더 많은 돈을 줘야만 하니까요. 하지만 정부는 이를 징병제로 징병한 의사들을 전국에 퍼진 공보의로 흩뿌리는 방식을 통해 싼 값으로 해결해 오고 있습니다. 이게 과연 징병제의 명분과 정확히 합치될 수 있는 것일까요? 징병제의 명분은 공동체의 구성원들은 공동체를 지키기 위해 일정 기간을 국가를 위해 복무해야 한다는 것입니다. 하지만 이것이 현실 정책이 되면, 국가는 공동체의 구성원들을 짧게는 1년 반, 길게는 3년 이상을 염가에 부릴 수 있다는 논리로 확장되고, 현재 국가는 보건의료, 법률상담 등의 고급 서비스를 염가에 제공함으로써 그 명분의 효과를 톡톡히 누렸습니다. 징병제 없이 어떻게 중소기업에서 설포카 대학원생들을 고용할 수 있으며, 시골 보건소에서 대위 월급만 주면서 의사를 굴릴 수 있고, 법률구조공단 같은 곳에서 정부 예산만 갖고 무료 법률상담 인건비를 감당하겠습니까? 각자의 입장에서는 군대를 가는 것보다는 3년 동안 굴림당하는 것이 낫고, 그렇다고 여호와의 증인처럼 징병제를 거부한다고 국가와 긴 법정다툼으로 들어가느니 그냥 3년 다녀오고 말자는 생각으로 대체복무를 택하게 되지만, 그 희생들을 모아 사회 전체는 고급 서비스를 염가에 제공하고 있는 것이죠. 그런데 이게 정말 지속 가능할까요? 앞서 징병제의 명분과 실제 정책이 차이가 있다는 점을 잠깐 말했는데, 징병제의 명분은 공동체의 안보에 있고, 안보를 담당하는 주무부처인 국방부는 현재의 인구 추세와 징병률로는 자신들이 생각하는 방위력을 달성하지 못한다는 생각을 갖고 지속적으로 그에 대비하고 있습니다. 예컨대 현역 판정률을 높이고, 안보에 필요없는 부분(PX관리 등)을 외주화하고, 후방 사단을 해체하여 전방 사단으로 돌리고, 기계화 사단 등으로 장비를 현대화하고, 사단 중심에서 여단 중심으로 바꾸는 등의 군제개편도 실시중이죠. 그리고 그 중 하나가 이공계 전문연구요원 폐지인 것입니다. 제 기억으로는 2016년 4월 경에 처음으로 국방부에게서 이공계 전문연구요원 폐지론이 나왔는데, 당시에는 과기부 등의 부처가 모두 반발하고 나서면서 국방부가 한 발 물러섰죠. 하지만 폐지의 정당성은 부처 간 파워게임과 상관없이 국방부에게 있는 것도 사실입니다. 과기부의 논리는 이공계 전문연구요원을 폐지하면 이공계 인력들이 2년여를 아깝게 자신의 역량을 키우지 못하고 낭비할 뿐더러 사회 핵심 인력들이 이공계를 지망할 인센티브를 없앤다는 건데, 이 말이 사회적으로는 분명히 참일 수 있겠으나 이공계 전문연처럼 그 사람들을 전용할 수 있는 권리 자체는 공동체의 안보를 근거로 하는 것이니까요. 그리고 결국 국방부가 이 방향대로 가면 결국 공보의나 공익 법무관 등에도 손을 댈 수밖에 없는데, 그렇게 된다면 그동안 한국이 누려 왔던 염가의 고급 서비스는 큰 위기를 맞이하게 될 것입니다. 그리고 이 위기는 그동안 사람들이 너무나도 당연하게 누려 왔던 금액들에 대한 도전이 될 것이구요. 그리고 이 비용에 대해 설명하고 그걸 감수하자고 하는 정치인들은 아무도 없죠. 다소 궤가 다른 이야기긴 합니다만, 저는 한국의 정책 기준처럼 통용되는 OECD 평균이라는 것도 싫어합니다. 각 사회는 그 사회에 맞는 특징을 갖고 있고 그 임금과 지출에는 그 사회 구조만의 특수성이 담겨 있는데 그걸 무시하고 단순히 <선진국 기준>이라는 용어로 접근하는 것은 위험하다고 느낍니다. 카톨릭 국가들이 갖는 모계사회의 특징들은 어린이집 같은 육아 외주비용을 절감하고, 프랑스의 바칼로레아가 한때 이상교육처럼 일컬어졌지만 프랑스의 에콜 노르말의 정관재계 엘리트 독점은 전성기의 서울법대와도 비교를 불허하는 수준이지요. 미국은 높은 사보험료 때문에 회사의 임금과 함께 회사에서 보장해주는 사보험이 임금보조 형태를 띄고 있고, 실리콘밸리의 프로그래머 초봉이 1억 2천이어도 그곳의 임대료는 월 500만원 수준이고요. 그럼에도 그런 사회의 결들을 모두 무시하고 북유럽 모델이니 프랑스 교육이니 하는 것들을 대한민국에 집어넣으려고 하면 마찰이 생길 수밖에 없겠지요. 한국의 최저임금제 논의를 다시 한 번 바라보면, 최저임금제는 분명히 '최소한의 인간다움을 영위할 수 있는 비용'임에도 불구하고 그냥 OECD 평균은 몇인가만 보았지, 그 임금의 기초가 되는 비용에 대한 논의는 철저히 배제되어 왔습니다. 같은 GDP를 갖고 있다고 해도, 의료비로 가구당 월평균 30만원을 지출하는 국가와 월평균 5만원을 지출하는 국가의 "인간답게 살 수 있는 임금"의 수준은 달라야겠죠. 그런데 결국 이런 세세한 계산보다는 "최저임금 1만원 시대"나 "최저임금 상승을 통해 경제성장을 할 수 있다"는 무리한 급진적 슬로건이 점유하는 걸 보고 있으면 저는 어느 공당도 진실을 마주할 용기가 없다고 생각합니다. 적절한 최저임금의 금액이란 결국 한국의 식비, 주거비, 교통비, 조세, 준조세, 의료비 등이 정확히 얼마 수준인가가 계산되어야 할 것입니다. 하지만 사회는 이 문제가 자신의 세대에서만 터지지 않으면 그만이라는 생각으로 폭탄 돌리기를 하고 있고 이걸 언제까지 개인의 희생으로 땜빵할 수 있을지도 의문이며 징병제 자체가 존속의 위기에 처한 상황에서 염가 서비스를 유지할 수 있을 지도 불확실합니다. 이번 한국전력의 대규모 적자를 정부가 예산으로 보조한다는 이야기를 들으면서 조삼모사朝三暮四라는 단어가 떠오르는 것은 저만이 아니겠죠. * Cascade님에 의해서 티타임 게시판으로부터 게시물 복사되었습니다 (2019-07-21 17:57) * 관리사유 : 추천게시판으로 복사합니다.
