| Subject | 건강한 노인들에게 저용량 아스피린을 장기 복용하면 어떻게 될까. |
 (미국에서는 알약이 500개가 들어있는 아스피린을 약국에서 일반인에게 팔고 있습니다.) 아스피린은 처음에는 해열/진통제로 개발되었지만, 미국의 시골이나 저개발국가를 제외하고는 아스피린을 해열/진통제로 사용하는 경우는 거의 없는 것이 현실입니다. 대신 심혈관계 질환에 있어서 경색을 예방하기 위해서 많이 사용하지요. 이 경우 저용량 아스피린을 사용합니다. 20세기 말부터 아스피린 및 비스테로이드성 진통소염제를 사용할 경우, 고형암을 예방하거나 고형암의 크기를 줄일 수 있다는 논문들이 나왔습니다. 그래서 아스피린을 건강식품처럼 먹으면 어떻게 될까라는 의문이 생겼습니다. 저용량 아스피린은 그동안 연구에서 복용을 통한 위험도 높지 않고, 심혈관 질환자들을 검사해서 위험이 있는 사람만 주는 것은 검사비용이 발생하니 그냥 심혈관질환 발생이 높은 고령의 환자들은 무조건 아스피린을 주면 심혈관질환도 예방하고 부수적으로 암도 줄일 수 있는 효과를 기대할 수 있을 것이라는 생각을 하게 되었죠. https://www.nejm.org/doi/full/10.1056/NEJMoa1805819 그래서 약 2만명의 노인을 대상으로 약 4.7년간 임상시험을 시행하여 보니, 아스피린을 복용한 군이 아스피린 대신 위약을 복용을 한 군과 비교하여 심혈관질환은 의미있게 감소하지 못하였고, 아스피린의 부작용인 위장관출혈과 같은 출혈은 의미있게 증가하였다는 결과를 발표했습니다. https://www.nejm.org/doi/full/10.1056/NEJMoa1803955 그리고 같은 저자들이 같은 자료를 이용하여 사망률을 분석한 결과, 아스피린을 복용하는 군이 사망률이 높았고, 암으로 인한 사망이 주 원인이어서 아스피린이 암을 억제한다는 그간의 속설도 기대할 수 없다는 것을 시사했습니다. 주의 : 이 글은 월도를 통해서 작성하였으므로 논문의 분석에 대해서 오류가 있을 수 있습니다. 사족 : NEJM에 연구자료를 잘라서 발표하다니, 연구 하나가지고 잘라서 논문 쓰지 말라고 한 사람들은 반성해야. 이 연구하나로 impact factor를 몇점을 획득했는지.... * Toby님에 의해서 티타임 게시판으로부터 게시물 복사되었습니다 (2018-10-10 01:15) * 관리사유 : 추천게시판으로 복사합니다.
