'사퇴'는 위기 상황 상기시키는 목적" (2.18, 오마이뉴스 인터뷰) "정부, 코로나 걸릴 테면 걸려보라 식…. 의료진 고통은 당연하다고 생각하나" (2.24, MBC 인터뷰) ""정부, 자꾸 안심하란 신호만..." 이재갑의 이유 있는 우려" (2.25, 오마이뉴스 인터뷰) 실제로 많은 분들이, 심지어 코로나에 걸린 분들조차도 오미크론 변이에 대해서는 그전의 변이들보다 가볍게 생각하실 겁니다. 정부의 주장과 세간의 이해처럼 오미크론 변이가 심각하지 않고 이것이 판데믹의 출구이자 '풍토병으로의 전환'이라면 예방의학 전문의들은 우려의 목소리를 왜 높이고 있는 것일까요? 첫 번째 이유는 현 확진자 증가세가 상당하다는 데에 있습니다. 보름이 지난 현재 COVID-19의 일간 확진자 수는 20만 명을 넘어섰습니다. 일 별 위중증자는 꾸준히 증가해 3월 7일, 1월 2일 이후 다시 1천 명을 넘겼습니다. 지난 3월 4일, 코로나로 인한 사망자는 216명으로 최고치를 찍었습니다. 한국은 전 세계에서 가장 많은 일일 확진자 수를 기록 중입니다. 한국의 백신 접종률은 3월 6일 기준으로 85.7%(약 4400만 명), 부스터 샷 접종률은 61.3%(약 3100만 명)로 인구 100만 이상 국가 중에서는 UAE, 포르투갈, 칠레, 중국, 쿠바, 싱가포르 다음으로 높습니다. 그런데도 오미크론 변이의 유행이 현재 한국을 유독 강타하고 있는 상태입니다. 두 번째 이유는 오미크론 변이에 관한 대중적 인식이 실제와 다를 수도 있다는 데에 있습니다. WHO 유럽 지부는 1월 19일 자로 공식 홈페이지에 〈오미크론 변이: 신화로부터 사실 구분하기〉(The Omicron variant: sorting fact from myth) 페이지를 개설합니다. 그만큼 오미크론에 대한 잘못된 믿음이 유럽에서도 널리 퍼지고 있었고 그걸 바로 잡아야 했습니다. 사실: 오미크론은 델타 변이보다 덜 심각해 보이지만, 온화한 것으로 보여서는 안 된다.. 신화: 오미크론은 가벼운 질병만 일으킨다. … 사실: 대유행의 종말이 아직 보이지 않는다. 신화: 오미크론이 덜 심각해짐에 따라, 우리는 대유행의 막바지에 다다르고 있다. 이 페이지의 볼드체 문구만 읽더라도 정부의 오미크론에 관한 설명이 WHO 유럽 지부의 설명과는 온도 차가 꽤 있습니다. 정부가 국민에게 불리한 사실을 숨기고 방역을 완화했던 것일까요? 2. 오미크론의 과학적 특성 우선은 오미크론 변이가 얼마나 심각한지를 조금 짚고 넘어갈 필요가 있습니다. 오미크론 변이에 관한 대부분의 초기 정보는 오미크론 변이가 가장 먼저 빠르게 확산하였던 남아공의 여러 연구로부터 얻어진 것입니다. 오미크론 변이가 이전의 변이보다 더 가볍다는 인상 역시 남아공의 민간 의료 보험사 디스커버리 헬스(Discovery Health)의 한 보고서로부터 시작되었습니다. 그 보고서는 오미크론 변이에 감염된 사람들 가운데 입원 위험이 이전 변이에 감염된 사람들과 비교해 29% 낮다는 내용이 담겨있었는데 이는 각 감염자별 초기 건강 상태, 기존 바이러스 감염 여부 등의 맥락이 제시되지 않은 기초 연구였습니다. (7, 8, 9) 한편 오미크론은 특별합니다. 이전의 우점종이라고 말할 수 있는 델타 변이까지의 바이러스들이 소위 계통 분류에서 서로 밀접한 관계에 있던 바이러스였던 것과 달리 오미크론은 그 진화 방향에서 멀리 있습니다. 오미크론까지의 바이러스 진화사를 밝히는 작업은 현재도 이루어지고 있으나 완전한 게놈 서열의 부족, 만성감염자의 존재, 인간 외 숙주의 가능성 등으로 인해 쉽게 알아내기 어려운 상태에 봉착해 있습니다. 그 때문에 오미크론은 단순 스파이크 단백질의 돌연변이 개수만 봐도 36개로 델타 변이의 9개보다 훨씬 많으며 원본-최초의 SARS-CoV-2 바이러스-와 비교했을 때 50개 이상의 돌연변이를 갖고 있습니다. 그리고 이 오미크론 돌연변이는 3개의 뚜렷한 하위 영역(BA.1,2,3)이 있습니다. 이런 돌연변이로 인해 오미크론은 감염(혹은 바이러스 입장에서 침투 및 확산)에 유리한 면역학적인 특성 두 가지를 갖고 있습니다. 하나는 면역회피특성이며 엔도좀 융합으로 변화된 세포 진입 방식입니다. 이 두 가지 특성으로 인해 오미크론은 불과 한 달 만에 델타 변이를 거의 대체했습니다. 정말 특이한 점은 원본 바이러스 역시 SARS에 비하면 약 20배 더 높은 ACE2 수용체 친화도를 지니고 있었는데 이 오미크론은 그 2019년의 바이러스보다도 훨씬 높은 수준의 친화도, 그만큼 높은 수준의 면역회피특성을 가지고 있다는 점입니다. 아주 많이 틀리게, 그러나 쉽게 설명하자면 오미크론은 델타보다 화력이 약한 대신 전파력이 좋습니다. 델타 변이에서 오미크론 변이로의 전쟁 상대의 변화는 COVID-19 대응 국면 역시 새로운 장에 접어들었음을 의미했습니다. 이미 12월 란셋의 코멘트, 「Omicron SARS-CoV-2 변종: COVID-19 전염병의 새로운 장」에서는 다음과 같이 말한 바 있습니다. ”새로운 변종으로 인한 백신 효능 저하에 대한 우려는 COVID-19 최종장에 대한 우리의 이해를 변화시켰고, 글로벌 백신 접종 그 자체로 SARS-CoV-2 감염을 통제하는 데 적합하다는 개념을 세계에서 부정한다.“ 지난 9월, 방역 당국이 펼치려다 실패한 위드 코로나 정책 역시 델타 변이의 유행성과 그에 대한 백신 부스터 샷의 효능에 기반한 접근이었습니다. 그러나 이제 상대는 델타가 아니라 오미크론입니다. 몇 달 사이 전쟁 상대가 델타 변이에서 오미크론 변이로 바뀌어 재검토해야 할 지점 중 하나는 백신입니다. 백신은 바이러스의 스파이크 단백질을 인식하는 중화항체를 면역세포에 만들어주는 기능을 합니다. 오미크론은 스파이크 단백질의 돌연변이가 많아서 앞서 언급한 란셋의 코멘트처럼 기존 백신이 어느 정도 효과 있을지에 대한 우려가 컸습니다. 3월 2일 영국의 역학 연구를 살펴보면, mRNA 백신을 두 번 맞았더라도 20주가 지나고부터는 오미크론 변이로 인한 증상 효과가 거의 나타나지 않았으며 부스터 샷으로 인한 효과 역시 크게 두드러지지 않았습니다. 물론 기본적으로 mRNA 백신이 일반 백신보다 더 효과적인 것으로 나타났습니다. 한편, 이스라엘에서는 4차 접종 후의 추이는 mRNA 백신이 3회 복용 후 면역성 천장에 도달함을 보여주었습니다. 이것은 4차 접종이 효과가 없다는 의미가 아니라 부스터 샷만큼 유의미한 중화항체 형성 증가를 가져오지 못했다는 말입니다. 영국의 연구처럼 오미크론 변이에 대해서는 그 효과가 더 적었습니다. 이런 상황에서 오미크론 백신의 필요성은 해외 공중 보건 전문가들 사이에서도 의견이 분분한 주제이며, 동물 연구에서 오미크론을 겨냥한 새로운 백신 부스터는 기존 백신 부스터 샷에 비해 어떤 이점도 제공하지 못하는 것으로 나타났습니다. 다시, 아주 많이 틀리게 그러나 쉽게 설명하자면, 오미크론에 대한 백신의 효과는 전보다 약해졌으며 부스터 샷과 4차 접종의 효과 역시 줄어들었습니다. 3. 방역 완화 논쟁과 정책 결정 이쯤에서 이재갑과 정부의 갈등을 보다 들여다보도록 하겠습니다. 이재갑은 사퇴를 통해 반정부적인 메시지를 강력히 전달하는 것처럼 보였으나 보이는 것처럼 오미크론 유행에 대한 역학적 차원의 인식 차이는 크지 않습니다. 그는 지금이 풍토병으로의 이행 단계로 접어들고 있다는 점에는 동의합니다. 그가 제기하는 문제는 한국적 맥락과 유행 관련 정부의 메시지에 있습니다. "우리가 외국처럼 이런 대규모 유행을 여러 번 겪었으면 또 한 번 겪는 거로 생각할 수 있다. 그런데 코로나 19, 2년 동안 이와 같은 대규모 발병 사례는 없었다. 국민도 준비가 안 되어있고, 정부도 준비가 안 되어 있다. (2.24, MBC 시선집중 인터뷰) "거리 두기를 완화할 수 없는 상황인데도, 완화하면서 심리적으로 오미크론이 별거 아니란 사인을 정부가 계속 주고 있다고 보거든요. 그것 자체가 지금 유행 상황이 악화되는 데 촉매제가 되고 있다고 생각합니다." (2.25, 오마이뉴스 인터뷰) 정부의 메시지에 대한 우려는 비단 이재갑만의 의견은 아닙니다. 오히려 이재갑은 전문가 집단 내에서는 친정부적 입장을 견지하고 있다고 봐도 무방합니다. 정부의 지침 다음날 나온 조선일보의 기사, 〈전문가들 “정점까지 3주 더 걸려…. 지금 방역 완화하는 건 무리”〉는 천은미, 엄관식, 김우주 등 많은 전문가가 애당초 ‘방역 완화’ 혹은 ‘출구의 초입’이라는 단어 선택, 그러니까 정부의 오미크론 유행에 대한 역학적 입장에 완강히 반대함을 드러내고 있음을 보여줍니다. 그러나 우리가 눈여겨봐야 할 것은 이 전문가 집단은 항상 우려의 목소리를 냈던 사람들이라는 점입니다. 지난 11월 29일 자 중앙일보의 기사, 〈"대선 표 걱정에 방역 포기" 재앙적 의료 현장, 우려 쏟아졌다〉 는 이러한 모습을 그대로 담고 있습니다. 자극적 제목은 무시하더라도 아직 델타 변이가 우세하며 확진자 수가 7천 명 선이던 당시에도 거리 두기 강화와 의료시스템의 붕괴에 대한 우려의 목소리가 계속 일었던 것입니다. 더 적지 않겠지만 COVID-19가 한국에 들어오고부터 꾸준히 ‘이렇게 해서는 망한다’라는 전문가들과 ‘이렇게 하면 된다’라는 정부 사이의 대립은 꾸준히 이어져 왔습니다.
그렇다면 전문가와 정부 중 누가 맞고 누가 틀린 걸까요? 정책은, 특히 다수의 생명과 직결된 의사 결정은 선진국들도 갈피를 잘 잡지 못하는 것처럼 보입니다. 원자력발전소 논쟁만 봐도 그렇습니다. 유럽의 원전 정책 패러다임은 불과 5년 만에 탈 정상과학 레토릭에 기반한 탈원전 기조에서 인류세 기후위기에 대응하기 위한 EU택소노미 하의 친환경적 원전 개발로 돌아섰습니다. 원자력발전소는 그 자리에 있습니다. 단지 실체적으로 그 자리에 있을 뿐만 아니라 원전의 안전성과 친환경성 역시 그대로입니다. 바뀐 것은 오로지 원전의 안정성과 친환경성을 둘러싼 가치 판단뿐이었습니다. 드라마 《체르노빌》과 연일 보도 되었던 후쿠시마의 모습은 과장된 환상이었을까요? 그렇지 않습니다. 영화 《돈룩업》은 단지 운석 충돌에 관한 실패한 코미디 혹은 기후위기에 관한 실패한 은유에 불과할까요? 그것 또한 아니라고 생각합니다. 다만 우리에게는 적, 혹은 관리대상, 혹은 고려요소가 더 늘어났을 뿐입니다. 이러한 협상의 대상 혹은 정치의 영역으로서 과학 정책은 단지 원자력발전소에만 국한되지 않습니다. EU는 기후위기 극복의 목적으로 미래 먹거리로 배양육 사업을 지원하기 시작했는데, 불과 5년 전만 하더라도 GMO(유전자 조작 식품) 재배에 반대하던 입장임을 고려하면 아주 큰 태도 변화입니다. 당연히 배양육이라고 해서 GMO에 대한 우려가 해제되지 않습니다. 다만 EU 내 GMO 식품에 대한 가치 판단의 무게추가 사전예방원칙에서 기후위기대응으로 급격히 옮겨갔을 뿐입니다. 4. 바람직한 정책 논의의 방향 다시 앞서 제기한 질문으로 돌아가 봅시다. 전문가와 정부 중 누가 옳은가? 둘은 다른 세상에 살고 있습니다. 전문가 집단 혹은 의사들은 의료 현장 최전선의 행위자들입니다. 그들은 매일 환자들을 피부로 접하고 그들의 앓는 소리를 직접 마주합니다. 지난 2년여간 방역 당국의 지침에 따라 비상 의료 체계를 유지해왔습니다. 정부 혹은 방역 당국은 방역 체계의 총책임자입니다. 그들은 매일 새로운 수치와 텍스트를 마주하며 끊임없이 미래를 예측하고 동시에 취사 선택해야 합니다. 지난 2년여간 방역 당국은 온갖 지역과 공간의 집단 감염을 마주하며 주 혹은 월 단위로 끊임없이 정책을 내야 했습니다. 이 둘의 의견일치가 일어난다면 그것은 오히려 정책의 실패를 의미할지도 모릅니다. 끊임없이 아픔을 체험하는 사람들과 수치화하는 사람들 사이에 공통적 합의가 발생하는 것은 적응, 한쪽의 일방적 포기 내지는 공모일 수 있습니다. 우리는 이미 토지 개발자들과 정책입안자들 사이의 작당 모의를 여러 번 마주한 바 있습니다. COVID-19를 둘러싼 여러 과학 논쟁들￣COVID-19의 위험성, 백신의 효과, 마스크의 기능 등￣은 별 탈 없이 합의가 이루어져 왔습니다. 이는 오미크론의 정체에 대해서도 대체로 그러합니다. 그러나 시간 축의 존재는 정책이 언제나 합리적일 수 없음을 명확히 합니다. 지난 1년 사이 COVID-19 우점종은 알파에서 델타로, 델타에서 오미크론으로 바뀌었습니다. 방역 당국이 사용할 수 있는 전략은 마스크 쓰기, 거리 두기, 백신 등으로 제한적인 와중에 한국은 이번 유행 전까지 효과적으로 COVID-19를 차단하고 있던 국가 중 하나였습니다. 현재 마주하는 유행은 여러 국가가 그동안 여러 번 마주했던 위기와도 같습니다. 이 말은 이중적 의미를 내포합니다. 그동안 큰 위기가 없었다는 말이기도 하며 동시에 현재 유행이 가져올 사회적 파급력을 쉽게 예측할 수 없다는 말이기도 합니다. 다른 국가들, 그리고 그 국민은 경험적으로 오미크론의 ‘경량화’를 마주하고 받아들일 수도 있겠으나 한국의 경우 다른 국면으로 전개될 수도 있다는 겁니다. 물론 이 유행은 지금까지 방역 당국이 주장해왔던 것처럼 전체적으로 잘 관리되어 넘어갈 수도 있습니다. 정책학은 다른 사회과학과 다르게 여지가 많습니다. 나쁘게 말하면 모호합니다. 정책학에서 다루는 많은 학자는 정책학자라기보다는 경제학, 윤리학, 정치학 등 다른 분과에 속해 있습니다. 이러한 특성은 정책 실제로부터 기인합니다. 정책은 설득입니다. 유관성(Relevance)을 따져야 하며 규범을 염두에 두어야 하며 정치 공동체로서의 행동이 어떠해야 하는지에 주목합니다. COVID-19 국면에서 방역 당국의 대응과 전문가 집단의 의견은 우리가 정책을 어떻게 접근해야 하는지를 선명하게 드러냅니다. 정치경제학자 윌다브스키는 정책 목표는 항상 복수형으로 상호모순적이며 끊임없이 변화하며 그 오류를 수정해나가는 것, 재설계가 정책의 정수라고 말합니다. 그리하여 정책 분석은 단지 특정 시점에서 성공과 실패를 재단하고 예측하기 위해 존재하는 것이 아닙니다. 정책분석은 선호의 변환에 관한 것이어야 합니다. 현재 대중의 선호가 어디를 향해 있는 지를 살피고 그 선호와 현 정책 사이의 거리를 확인 후 도달 가능한 지점을 제시하는 것. 그것이 바람직한 정책 논의의 방향이 될 것입니다. * Cascade님에 의해서 티타임 게시판으로부터 게시물 복사되었습니다 (2022-03-22 09:02) * 관리사유 : 추천게시판으로 복사합니다.
