검색해보니 유형으로 1) 조기 포만감, 더부룩하며 가스가 차고 갑갑함이 주 증상이 되는 위 운동장애군 2) 속이 비면 쓰리고 아프다가 음식을 먹으면 가라앉는 마치 소화성 궤양과 유사한 증상을 가지는 궤양 유사증상군 3) 가슴에 뜨거운 작열감이 있고 신물이 올라오는 역류성 식도염 유사증상군 세 가지가 있던데 이 중 1번과 3번을 갖고 있습니다. 병원에도 2달 가까이 다니며 내시경 및 몇몇 검사를 통해 특별한 질환이 없는 걸 확인했고, 약을 받아먹었는데 약을 먹을 때도 별 변화가 없었으며 같은 증상을 계속 느꼈습니다. 약을 먹을 때는 약간 덜했던 것 같기도 합니다만 같은 증상을 약을 먹으면서도 반복했던 건 분명합니다. 마지막으로 헬리코박터 검사를 해보자 하셔서 해봤으나 이 또한 아니었고 선생님께서는 이제 포기를 하셨네요. 이젠 오지 말고 더 해보려면 큰 병원에 가보라고 하십니다. 몸이 좋지 않으니 몹시 우울하고 슬픕니다. 밥을 먹지 못 하겠고 흡사 거식증처럼 됐네요. 배가 고프지만 밥을 먹으면 쓰리거나 또는 계속 트림을 할 걸 생각하니 먹기가 싫습니다. 의사 선생님께서도 포기를 하셨다 생각하니 더욱 우울하네요. 앞으로 어떻게 하면 좋을까요? 1.대학병원에 가본다. -기능성 소화불량에 대해 대학 병원에서는 특별한 방법이 있나요? 딱히 병변이 없이 발생하기에 기능성 소화불량이라고 부른다고 알고 있는데 그렇다면 대학 병원에서도 뾰족한 방법이 없을 것 같아서요. 사실 가난해서 대학 병원은 걱정이에요. 2.채식을 하며 산다. -육류를 좋아하니 이것도 우울한 방법이긴 합니다만 경험상 기름진 고기보다는 채소류를 먹을 때 증상이 덜한 것 같았습니다. 그렇다면 채식을 하는 게 도움이 되지 않을까 하는 생각도 드네요. 다만 지금도 너무 힘들어서 고기는 잘 먹지 않는데도 증상에 큰 차이가 없어서 기대가 안 되네요. 채식을 해도 곡물은 먹을 건데 곡물도 썩 소화가 잘 되는 것 같지는 않아서요. 3.운동을 열심히 한다. -어머니께서 운동이라도 해보라 하시네요. 막연하지만 운동을 해서 체력이 좋아지면 소화계도 좀 건강해지지 않을까 하는 생각도 듭니다. 지금도 자전거는 열심히 타고 있는데 ㅜㅜ 4.소화제를 매 끼니 먹는다. -이것도 마치 병자가 되는 것 같아 생각하는 것만으로도 몹시 우울한데 만성적으로 소화불량인 분들은 소화제를 집에 상시 구비하고 계시기도 하니 이런 것도 괜찮을까요? 다만 이 쪽은 제가 소화제의 약효를 겪어보지 못 한 점이 걸립니다. 까스활명수나 병원에서 처방해준 것밖에 먹어보지 못 하긴 했지만 그것들 먹었을 때 뚜렷한 약효는 못 봐서요. 갤포스 비슷한 끈적한 액상의 약들은 먹었을 때 속쓰림을 막아주긴 했습니다. 근데 더부룩함에 대해서는 아직 약으로 실효를 보질 못 했네요. 속쓰림만 막아도 좀 낫기는 하겠네요. 오늘 속쓰려 죽을 뻔 ㅜㅜ... 어떻게 하면 좋을까요? 1. 흉부 작열감이 있고 이전 문의글에서 역류성 식도염이 있으셨다고 하니, 마음같아서는 24시간 산도 검사(정확한 한글 이름이 이게 맞는지는 모르겠네요..)를 해보시라고 권유하고 싶습니다. 전극을 식도에 집어넣는 검사인데, 대단히 불편해서 사실 잘 안하는 검사입니다. 증상이 있을 때 역류가 있는지 확인해볼 수 있습니다. (하부 식도의 pH를 검사해서, 증상이 있을 때 산성으로 떨어지는지 볼 수 있어요. 만약 증상과 매칭이 된다면,.. 원인으로 볼 수 있겠죠 ?)

1. 흉부 작열감이 있고 이전 문의글에서 역류성 식도염이 있으셨다고 하니, 마음같아서는 24시간 산도 검사(정확한 한글 이름이 이게 맞는지는 모르겠네요..)를 해보시라고 권유하고 싶습니다. 전극을 식도에 집어넣는 검사인데, 대단히 불편해서 사실 잘 안하는 검사입니다. 증상이 있을 때 역류가 있는지 확인해볼 수 있습니다. (하부 식도의 pH를 검사해서, 증상이 있을 때 산성으로 떨어지는지 볼 수 있어요. 만약 증상과 매칭이 된다면,.. 원인으로 볼 수 있겠죠 ?)
2. 그런데 말씀드렸다싶이 너무너무 불편한 검사다보니, 잘 안합니다.. 그래서 보통 약물을 투여해봐서 호전이 있는지 보게 됩니다. 혹시 ppi라는 약을 드셔보신 적이 있나요 ? proton pump inhibitor라는 약인데, 드셨을 때 호전 여부가 중요합니다. 특히 약 기전상 식전에 먹는 것이 중요합니다. 만약 제 때 복용하였을 땨 증상이 호전되는게 보인다면, 지속적인 투여로 호전을 기대해볼 수 있습니다. 다만, 아침 식전에 먹는 약니다보니 낮에는 괜찮다가 밤이나 새벽(약빨이 왠지 떨어질때쯤 ??)녘에 증상이 심해지는 것 같다면.. 저녁 식전에 한 번 추가하면 증상 호전에 도움될 수 있습니다. 문제는 약 먹고 좋아졌더라도, 약 끊으면 쉽게 재발한다는거..
3. 그 외 원인에 대해서도 생각을 해봐야 하는데, 내시경상 역류성 식도염 외에 특별한거 없으셨다고 하니.. 혹시 술 담배를 하시는 중이라면 우선 꼭 끊으시기 바랍니다. 비만이시라면 체중 감량도 도움이 될 것입니다.
4. 혹시 설사를 많이 한다던지, 가끔씩 얼굴이 화끈화끈 뻘개지거나 올라오는 경우가 있는지.. 구토가 동반되는지.. 소화가 안되는 증상이 토하면 호전이 되는건지.. 등도 중요합니다.
